中药联合阿卡波糖对糖耐量受损者脂联素及胰岛功能的影响

摘要 目的观察中药治疗联合阿卡波糖对糖耐量受损(IGT)患者血糖素及胰岛功能的影响,为临床提供治疗依据。方法选取2013年9月至2014年10月我院收治的62例患者作为研究对象,随机均分为对照组与实验组,对照组给予阿卡波糖,实验组在对照组的基础上给予中药治疗,疗程10w,治疗前后监测两组的脂联素、FBG,2h PG、空腹胰岛素(FINS)、2 h血清胰岛素(2h INS)、糖化血红蛋白(HBA1c),并计算胰岛素抵抗指数(HOMA-IR),评价两组患者的脂联素与胰岛功能的变化。结果治疗后,两组脂联素均升高,但实验组脂联素水平高于对照组(P<0.005);两组FINS、2h INS与HOMA-β水平升高,但实验组优于对照组(P<0.05),提示实验组胰岛分泌功能改善更加明显(P<0.05);两组FBG、2h PG与HBA1c水平均下降,但实验组优于对照组(P<0.05),提示实验组胰岛血糖调节功能改善更佳(P<0.05)。结论中药治疗联合阿卡波糖能够显著改善IGT患者的胰岛功能,这与脂联素表达增强与药物的直接协同作用密切相关。 Objective To study the effect of Ttraditional Chinese medicine (TCM) combined with acarbose on the level of adiponectin and islet function in impaired glucose tolerance, so as to provide treatment basis. Methods 62 cases of IGT patients in our hospital from sep. 2013 to oct. 2014 were selected as research subjects and randomly divided into the control group and the experimental group. The control group was treated by acarbose, but the experimental group was given the TCM on the basis of the control group. The treatment course lasted for 10 weeks. The adiponectin, FBG,2hPG,FINS,2hINS and HBAlc in the two groups were determined before and after treatment, the HOMA - IR was calculated and the changes of adiponectin and islet function were evaluated in the two groups. Results After treatment, the level of adiponectin became higher in the two groups, but the level of adiponectin in the experimental group was higher than that in the control group (P 〈 0.05 ). The level of FINS,2hINS and HOMA - β became higher in the two groups, but the experimental group was better (P 〈 0.05 ), suggesting the islet secretion of the experimental group more significantly improved (P 〈 0.05). The level of FBG,2hPG and HBAlc became lower in the two groups, but the experimental group was better (P 〈 0. 05), suggesting the regulating function of islet blood glucose more significantly improved (P 〈 0.05 ). Conclusion TCM combined with acarbose can more efficiently improve the islet function of IGT by enhancing the expression of adiponectin and direct effects.
