摘要
目的分析报道由母源性HLA抗体引起的新生儿同种免疫性血小板减少症。方法以4名临床上确诊为新生儿免疫性血小板减少症(NAIT)的患儿为研究对象,采用ELISA方法检测患儿及其母亲血浆中的血小板特异性抗体和血小板自身抗体;应用PCR-SSP的方法对患儿及其父母进行HPA/HLA基因分型;借助Luminex检测平台进行HLA抗体特异性检测分析。结果 4名患儿及其母亲均未检出血小板特异性抗体和自身抗体,然而存在与胎儿父源性HLA抗原相对应的HLA特异性抗体。结论对临床上怀疑发生NAIT的胎儿,实验室血小板抗体检测除考虑HPA抗体外,还应考虑HLA抗体的因素。
Objective To analyze and report cases of neonatal alloimmune thrombocytopenia associated with maternal HLA antibodies. Methods Four children clinically diagnosed as neonatal alloimmune thrombocytopenia were included in this study. Platelet alloantibodies and autoantibodies of patients and their maternal plasma were measured by ELISA method. Using PCR-SSP, the HPA/HLA genotype of patients and their parents were analyzed. The specificity of HLA antibodies was also tested with the Luminex detection platform. Results No platelet-speeific antibodies and autoantibodies were found in the plasma of four children and their mothers. However, HLA antibodies, which corresponded to paternal HLA antigen, were found in their sera. Conclusion For fetuses suspected with NAIT clinically, platelet antibody testing in Lab should also consider HLA antibodies factors while excluding HPA antibodies.
