摘要
目的分析研究非传染性输血不良反应的发生原因,寻求快速诊断方法,为及时救治赢得时间,提高输血疗效。方法采用从输血不良反应发生前、中、后三阶段为"一体"的非传染性输血不良反应快速鉴定法,对济源市2015年5月-11月发现发生的非传染性输血不良反应病例进行病因分析、不良反应类型诊断和后继输血策略指导,探讨此快速鉴定法的实用性和可行性。结果发现发生输血不良反应42例,其中非溶血性发热反应13例,过敏3例,过敏伴发热3例,荨麻疹5例,红细胞同种免疫5例,HLA抗原同种免疫11例,铁超负荷2例;5例非溶血性发热反应和3例过敏反应患者在后续输血选择洗涤红细胞,未再发生不良反应。结论快速鉴定法能快速识别输血不良反应临床症状体征、分析诊断不良反应类型,尽早采取相应救治措施,是一种简便、快捷和有效的鉴定方法,值得推广应用。
Abstract:Objective To investigate the causes of non infectious transfusion adverse reactions, to investigate a quick diagnosis method, to achieve timely treatment and to improve the efficacy of blood transfusion. Methods The rapid identifi- cation method of non infectious transfusion adverse reaction was adopted from the stages of the adverse reaction of blood trans- fusion before, during and after the three stages. The causes of non infectious transfusion adverse reactions occurred in Jiyuan city from May 2015 to November were analyzed. Diagnosis of adverse drug reaction type was conducted and subsequent blood transfusion strategy guidance were provided. The practicability and feasibility of this method were discussed. Results A- mong the 42 cases of adverse reactions, 13 cases were non hemolytic febrile reactions, allergy in 3 cases, urticaria in 5 ca- ses, red blood cell immune response in 5 cases, 11 cases of HLA antigen of the same kind of immune response, 2 cases of iron overload, 5 cases of non hemolytic febrile reactions and 3 cases of allergic reactions in patients with subsequent blood transfusion with washed red blood cells but experienced no adverse reactions. Conclusion Rapid identification method can quickly identify the clinical signs and symptoms of transfusion adverse reactions, analyze and diagnose adverse reaction types. Appropriate treatment measures should be taken. This is a simple, fast and effective identification method and is worth promotion.
