摘要
目的:比较中医及西医健康相关生活质量量表在测量结构层次和条目层次上概念的异同。方法:前期系统评价纳入具有较好信度、效度的中、西医健康相关生活质量量表,重新形成条目池;应用内容分析法对条目的内容进行客观、系统的描述:参考WHO和PROMIS定义测量结构(包括维度、亚维度、概念、特点);3位小组成员按照定义对条目进行独立编码,达成共识并将相同条目归类,进而构建新模型。通过对比新模型与原有模型和对比新模型下中、西医条目概念的异同,来比较中、西医量表测量结构和条目概念的异同。结果:研究纳入4个中医量表和2个西医量表,建立包含295个条目的条目池,中医条目占54%。新模型含5个维度、11个亚维度和68个概念。97%原有模型条目与新模型相似(包括完全和部分相似),其中包括100%西医和96%中医条目。新模型中,相同测量结构下,中医和西医的相同概念占55%,中医量表42%的条目与西医71%的条目概念相似;此外,中医条目有58%为特色概念,西医有29%为特色概念。结论:中、西医对健康相关生活质量概念的测量结构基本相似,而中、西医的差异可能集中在具体条目的测量概念。
Objective： To compare the similarities and difference in concept of health related Quality of Life（hr-QOL） based on the measuring frames and items of Traditional Chinese medicine（TCM） and modernized medicine（MM） measures. Methods： Previous systematic review has included TCM and MM hr-QOL measures with good psychometric properties. Items from the both medicines＇ measures were pooled and analyzed by the content analysis objectively and systematically. Three members separately coded the items according to the definition on the measuring frame of WHO and PROMIS, and the consensus was reached by the group discussion. After that, the items were categorized by the codes and aggregated in the new measuring model. The similarities and differences between TCM and MM＇s measuring frames and concepts were analyzed by comparing new model with original model of 6 measures and comparing items＇ concepts underlying the same frames. Results： From four TCM and two MM measures, 295 items have been pooled, where TCM items＇ proportion was 54%. New model included five domains, 11 sub-domains and 68 concepts. The original measures showed 97% totally and partly similar with the new ones, including 100% MM items and 96% TCM items. In the new model, underlying the same measuring frames, proportion of 55% were similar concepts of TCM and MM, where 42% were TCM items and 71% MM items. Besides, 58% of TCM items have TCM specific concepts, and 29% of MM items have MM specific concepts. Conclusion： The TCM and MM concepts are mostly similar on the measuring frames, and majority of differences focus on the items＇ concepts.
