摘要 目的分析手术室内环境对医护人员的影响及防护对策。方法 2016年6月调查哈尔滨医科大学附属第一医院手术室医护人员300名过去1周内工作期间产生的不适感,调查问卷采取自行填写的方式,统计不同手术室面积、麻醉气体暴露、手术室内消毒方式、刺激性气体暴露的医护人员出现不适感的比例和医护人员出现不适症状后采用的缓解方式,从而建立保障手术室医护人员健康的有效措施。结果手术室面积20、30、40 m2医护人员出现不适感的发生率分别为82.11%、74.17%、78.82%。安氟烷、七氟烷+氧化亚氮、地氟烷暴露导致医护人员出现不适感的发生率分别为100.00%、90.91%、80.95%,占麻醉气体暴露前3位,明显高于异氟烷+氧化亚氮、七氟烷、异氟烷。甲醛熏蒸法+循环风紫外线消毒机、紫外线灯+循环风紫外线消毒机、循环风紫外线消毒机出现不适感的发生率分别为86.84%、82.35%、78.65%,明显高于臭氧+循环风紫外线消毒机、多功能动态杀菌机、其他导致医护人员出现不适感的发生率。放射线+手术操作时仪器烧灼气体+麻醉废气、化学消毒剂+手术操作时仪器烧灼气体+麻醉废气出现不适感的发生率分别为84.83%、84.78%,占刺激性气体前2位,明显高于麻醉废气+化学消毒剂、化学消毒剂、其他导致医护人员出现不适感的发生率。医护人员选择呼吸新鲜空气、脱离环境来缓解不适感的比例分别为33.33%,29.00%,占缓解方式前2位,明显高于选择短暂休息、药物治疗及吸氧来缓解不适感的比例。结论手术室医护人员出现的不适感与手术室环境相关,包括手术室面积、麻醉气体、消毒方式等,可通过建立有效的防护对策以达到降低此类不适感的目的 ,从而提高工作质量。 Objective To analyze the influence of operating room environment on medical staffs and its protective countermeasures. Methods The discomfort of the medical staffs working in Operating Room of the First Affiliated Hospital of Harbin Medical University during the past 1 weeks was recorded by using the method of questionnaire in June 2006. The discomfort rates of medical staffs working in different operating room areas, anesthetic gas exposure,operating room disinfection methods, excitant gas exposure and the relieve methods taken by the staffs had discomfort symptoms were analyzed, in order to establish effective measures to protect the health of operating room staffs. Results When the operating room area was 20, 30 and 40 m2, the incidence of discomfort of the medical personnel was 82.11%,74.17% and 78.82% respectively. The incidence of discomfort of medical staffs caused by Ann Halothane, Sevoflurane+ Nitric Oxide, Halothane was 100.00%, 90.91%, 80.95% respectively, accounted for the top three anesthetic gas exposure, which were higher than the incidence of discomfort caused by Isoflurane + Nitric Oxide, Sevoflurane,Isoflurane. The incidence of discomfort caused by formaldehyde fumigation + wind circulation ultraviolet disinfection machine, UV lamp + wind circulation ultraviolet disinfection machine, circulation wind ultraviolet disinfection machine were 86.84%, 82.35%, 78.65% respectively, which were higher than the incidence of discomfort caused by by ozone +wind circulation ultraviolet disinfection machine, multi-functional dynamic sterilization machine and others. The incidence of discomfort caused by radiation + operation instrument burning gas + waste anesthetic gas, chemical disinfectant + operation instrument burning gas + waste anesthetic gas were 84.83%, 84.78% respectively, account for the top two irritant gas, which were higher than the incidence of discomfort caused by waste anesthetic gas + chemical disinfectants, chemical disinfectants and others. The rates of medical staffs chosen breathing fresh air, breaking away from environment to relieve the discomfort were 33.33%, 29.00% respectively, accounting for the top two relief ways,which were significantly higher than choosing short breaks, drug treatment and oxygen therapy to relieve the discomfor.Conclusion The discomfort of medicine staffs in operating room are associated with operating room environment,including the operating room area, anesthetic gasses, disinfection methods, etc. Effective protective measures should be establishmented to achieve the goal of reducing such discomfort, so as to improve the work quality.
