摘要
目的：探讨积极心理干预与家庭系统治疗联合应用于老年痴呆症患者护理中的效果。方法选择2013年5月—2015年10月南京大学附属鼓楼医院收治的老年痴呆症患者114例,随机分为两组,对照组（n＝57）接受传统护理干预,观察组（n＝57）接受积极心理干预联合家庭系统治疗,干预前后采用简单精神状况检查量表（ MMSE）、汉密尔顿抑郁量表（ HAMD）、日常生活能力表（ ADL）和家庭功能评定量表（ FAD）比较两组干预效果。结果干预前两组患者认知能力评价、HAMD评分、ADL评分、FAD评分差异无统计学意义（ P〉0.05）；干预后,观察组认知能力评价各维度得分高于对照组,HAMD评分低于对照组,ADL评分高于对照组,FAD各项评分高于对照组,差异均有统计学意义（P〈0.05）。结论将积极心理干预与家庭系统治疗联合应用老年痴呆症患者的护理中,可以提高患者的生活质量,改善患者的认知功能,对临床治疗有一定的辅助作用,建议推广使用。
Objective To explore the clinical effect of positive psychological intervention combined with family systemic therapy in patients with Alzheimer′s disease.Methods Totally 114 cases with Alzheimer′s disease that admitted in our hospital from May 2013 to October 2015, were selected as the research object and divided into two groups according to random number table method. 57 cases in the control group were given the routine nursing intervention and another 57 cases in the experimental group were given the positive psychological intervention combined with family systemic therapy. Mini-mental state examination （ MMSE ） , Hamilton depression scale （HAMD） and the activity of daily living scale （ADL） and family assessment device （FAD） were used to investigate the patients, and the results were compared in two groups. Results Before the intervention, there is no difference in the evaluation of cognitive ability, HAMD score, ADL score and FAD score between two groups （ P〉0.05） . After the intervention, HAMD score in the experimental group was lower than that in the control group;the score of dimensions in cognitive ability evaluation, the ADL score and FAD score were higher than that in the control group （P〈0.05）.Conclusions The positive psychological intervention combined with family systemic therapy applied in patients with Alzheimer′s disease can improve the quality of life and cognitive function. It also has auxiliary function to clinical treatment, and it is recommended.
