摘要
目的:探讨综合干预措施对南通市第一人民医院(以下简称"我院")Ⅰ类切口手术围术期预防性应用抗菌药物的影响,为提高抗菌药物合理应用水平提供参考。方法:我院于2015年第2季度开始实施等级医院复核评审综合干预措施。抽取我院2014年第3、4季度(干预前,851例)和2015年第3、4季度(干预后,791例)出院的甲状腺、乳腺、腹股沟疝及白内障等4种Ⅰ类切口手术患者共1 642例的相关资料,对干预前后抗菌药物预防性使用率、品种选择、术前给药时间、用药疗程等指标进行对比分析和统计学处理,综合评价干预效果。结果:我院Ⅰ类切口手术围术期抗菌药物预防性使用率由干预前的53.5%降至干预后的14.2%,差异有统计学意义(P<0.05);无适应证预防性使用抗菌药物的比例由干预前的85.9%降至干预后的72.3%,差异有统计学意义(P<0.05)。结论:我院采取切实可行的行政和技术干预措施,有效降低了Ⅰ类切口手术围术期抗菌药物预防性使用率,提高了抗菌药物合理应用水平。
OBJECTIVE: To investigate the effects of comprehensive intervention on prophylactic application of antibiotics in type Ⅰ incision during perioperative period in Nantong the First People's Hospital( hereinafter referred to as "our hospital "),so as to provide reference for the improvement of level of rational drug application in clinic.METHODS: Comprehensive intervention in graded hospital was implemented from the 2nd quarter of 2015. 1 642 cases with type Ⅰ incision as thyroid surgery,breast surgery,inguinal hernia surgery and cataract surgery during the 3rd( before intervention,851 cases) and 4th( after intervention,791 cases) quarter of 2014 and 2015 were extracted.Statistical analysis and comparison were conducted on the prophylactic application rate and categories selection ofantibiotics,administration time before surgery, treatment course, the intervention effects were comprehensively evaluated. RESULTS: The prophylactic application rate of antibiotics in type Ⅰ incision decreased from 53. 5% before intervention to 14. 2% after intervention,with statistical significance( P〈0. 05). The ratio of no indication of prophylactic application of antibiotics decreased from 85. 9% before intervention to 72. 3% after intervention,with statistical significance( P〈0. 05). CONCLUSIONS: The feasible administrative and technical interventions adopted by hospital can effectively decrease the prophylactic application rate of antibiotics in type Ⅰ incision,and improve the level of rational application of antibiotics.
