摘要
目的比较磁共振成像(MRI)与X线计算机断层摄影(CT)在老年多发性脑梗死病人诊断中的临床结果。方法选择2013年9月至2015年9月收治的93例老年多发脑梗死患者的临床资料实行回顾性分析,让全部患者均接受MRI与CT检查诊断,并对比两种方法在不同发病时间、不同大小的梗死病灶、不同体积的梗死灶以及血管性痴呆与非血管性痴呆的诊断效果。结果 MRI在24 h内发病患者的阳性诊断率为93.55%,高于对照组的51.61%,差异具有显著性(P<0.05);MRI检测出的病灶总数为367个多于对照组的227个,且小病灶检测率和基底节检测率均高于对照组,差异具有显著性(P<0.05);2 mm以内的梗死灶,MRI检出阳性例数明显高于CT检出阳性例数(P<0.05),>2 mm的梗死灶,CT和MRI检出的例数无显著性差异(P>0.05);非血管性痴呆组患者的MRI对梗死的检出率明显高于CT法(P<0.05),而对血管性痴呆患者的梗死阳性检出率两种方法无显著性差异(P>0.05)。结论将MRI应用在老年多发性脑梗死病人的诊断中,不仅能够获得较高阳性诊断率,而且能够全面检测患者的病灶个数,同时对多发脑梗死痴呆的梗死病灶检出率明显高于CT,因此MRI的检测具备更大的应用优势。
Objective To compare MRI and CT clinical outcome in elderly patients with the diagnosis of multiple cerebral infarction. Methods 93 cases of elderly patients with multi - infarct clinical data were analyzed retrospectively imposed form September 2013 to September 2015, so that all patients underwent MRI and CT diagnostic. And compared the two methods in different time of onset, different sizes of infarction lesions, different volume of infarction and vascular dementia and non - vascular dementia diagnosis. Results The positive diagnosis rate of MRI on onset cases at 24 h was 93.55% , which is higher than that of control group (51.61% ) with significant difference ( P 〈 0.05 ). The total number of lesions detected by MRI was 367, which was more than that of the control group ( 227 ) , and the detection rate of small lesions and basal ganglia lesions was higher ( P 〈 0.05 ). The detection rate of MRI on lesions with diameter ≤2 mm was significantly higher than that of CT( P 〈 0.05 ) , but there was no difference on the detection rate of lesions 〉 2 mm ( P 〉 0.05 ). The detection rate of non vascular dementia patients was significantly higher than that of CT ( P 〈 0.05 ) , but there was no difference on vascular dementia patients ( P 〉 0.05 ). Conclusion The performance of MRI on elderly patients with the diagnosis of multiple cerebral infarction, not only can obtain a higher rate of positive diagnosis, but also can fully detect more lesions in patients. At the same time, the detection rate of infarction lesions in multiple infarct dementia is significantly higher than that of CT, so the detection rate of MRI has a greater advantage.
