摘要
目的:观察外源性骨诱导生长因子注射用骨瓜提取物治疗老年桡骨远端骨折的临床疗效。方法:收集2013年1月-2015年1月某院骨科收治的老年桡骨远端骨折的患者104例,将其分为2组.治疗组(n=54)给予50 mg注射用骨瓜提取物加入250 mL0.9%氯化钠注射液静脉滴注,对照组(n=50)给予碳酸钙口服,均治疗1-3个疗程,观察2组患者骨折愈合效果和治疗前后骨代谢改变情况。结果:治疗2个月后,治疗组健侧桡骨骨密度(0.53±0.06)g/cm^2显著高于对照组(0.49±0.03)g/cm^2,差异有统计学意义(t=4.301,P〈0.05);骨钙素水平(5.34±4.87)μg/L显著高于对照组(2.21±0.84)μg/L,差异有统计学意义(t=3.982,P〈0.05);血清骨碱性磷酸酶(145.72±30.28)显著高于对照组(114.29±37.43),差异有统计学意义(t=5.327,P〈0.05);I型前胶原竣基端肽(5.98±1.74)μg/L显著高于对照组(3.97±1.28)μg/L,差异有统计学意义(t=3.694,P〈0.05)。治疗组临床有效率(87.04%)显著高于对照组(44.00%),差异具有统计学意义(t=4.301,P〈0.05)。结论:外源性骨诱导生长因子注射用骨瓜提取物治疗老年桡骨远端骨折具有较好的临床效果,可以明显改善患者的骨代谢情况。
Objective: To observe the effect of exogenous induced bone growth factor Gugua extract injection in treatment of distal radius fracture of the elderly. Methods: 104 elder patients with distal radius fracture were collected from the department of orthopedics of our hospital between January 2013 and January 2015,whom were divided into two groups according to the willing of them and their family members. The treatment group( N = 54) was given 50 mg Gugua extract injection with 250 ml 0. 9% sodium chloride injection by intravenous drip,while the control group( N = 50) was given oral administration of calcium carbonate. Both groups were treated for 1-3 periods of treatment. The effect of fracture healing and changes of bone metabolic before and after the treatment were observed. Results: After 2 months of treatment,the contralateral radial bone mineral density of the treatment group( 0. 53 ± 0. 06) was higher than that of the control group( 0. 49 ± 0. 03),with significant difference( t = 4. 301,P〈0. 05); the bone calcium level of the treatment group( 5. 34 ± 4. 87) was higher than that of the control group( 2. 21 ± 0. 84),the difference being statistically significant( t = 3. 982,P〈0. 05); the serum bone alkaline phosphatase of the treatment group( 145. 72 ± 30. 28) was higher than that of the control group( 114. 29 ± 37. 43),with statistically significant difference( t = 5. 327,P〈0. 05); type Icollagen stern of the treatment group( 5. 98 ±1. 74) was higher than that of the control group( 3. 97 ± 1. 28),the difference being statistically significant( t = 3. 694,P〈0. 05).The clinical effect of the treatment group( 87. 04%) was higher than that of the control group( 44. 00%). The difference was statistically significant( t = 4. 301,P〈0. 05). Conclusion: Exogenous growth factor Gugua extract injection has good clinical effect on distal radius fracture of the elderly. It can obviously improve the patient's bone metabolism.
