摘要
目的观察调和解毒法联合四联疗法治疗幽门螺杆菌阳性消化性溃疡的临床疗效。方法将96例幽门螺杆菌阳性消化性溃疡患者随机分为对照组46例和研究组50例。对照组给予奥美拉唑、克拉霉素、阿莫西林、胶体果胶铋四联疗法治疗,研究组在对照组治疗基础上给予根据调和解毒法制定的基本方随证加减治疗,2组均治疗8周。观察2组治疗前后中医证候积分变化情况,统计2组中医证候疗效、胃镜疗效及幽门螺杆菌清除、根除情况和不良反应发生情况。结果治疗后2组中医证候积分均较治疗前明显降低(P均<0.05),且研究组明显低于对照组(P<0.05);研究组中医证候总有效率和胃镜下总有效率均明显高于对照组(P均<0.05);2组治疗后幽门螺杆菌清除率比较差异无统计学意义(P>0.05),但研究组根除率明显高于对照组(P<0.05);研究组不良反应发生率明显低于对照组(P<0.05)。结论调和解毒法联合四联疗法治疗幽门螺杆菌阳性消化性溃疡可有效提高中医证候疗效和胃镜疗效,提高幽门螺杆菌的根除率,降低中医证候积分和不良反应发生率,具有很好的临床应用和推广价值。
Objective It is to observe the clinical effect of applying method of harmonizing liver and spleen, relieving of toxicity combined with quadruple therapy for treating H. pylori positive peptic ulcer. Methods 96 patients with H. pylori posi- tive peptic ulcer were randomly divided into two groups ： control group （ n = 46） and study group （ n = 50）. The control group was treated with Omeprazole capsules,clarithromyein Sustained-release tablets, amoxicillin capsules and colloidal bismuth pec- tin capsules. The study group was treated with Chinese medicine of harmonizing liver and spleen, relieving of toxicity based on quadruple therapy of control group. All patients were treated for 8 weeks. Clinical symptoms, gastroseope curative effect, the degree of clearing and eradicating helicobaeter pylori and adverse drug reaction were used to assess the clinical effect after treatment. Results The Chinese medical syndrome scores were decreased significantly after treatment compared with that before treatment in both groups （ P 〈 0.05 ） , and the decrease in study group was more significant than that in control group. The tra- ditional Chinese medical syndrome total effective rate and effective rate of gastroscope image were obviously higher in study group than that in control group（ P 〈 0.05 ）. After treatment, there was no difference in clearance rate of H. pylori between the two groups （ P 〉 0.05 ） , but the eradication rate in study group was obviously higher than that in control group （ P 〈 0.05 ）. The occurrence rate of adverse reaction in study group was distinctly lower than that in control group （ P 〈 0.05 ）. Conclusion The method of harmonizing liver and spleen,relieving of toxicity combined with quadruple therapy in treatment of helicobacter pylori positive peptic ulcer was effective on improving the curative effect of TCM syndrome and gastroscope image, improving the eradication rate of H. pylori, reducing the integral of TCM symptom and the occurrence rate of adverse reaction, which has nicer practicality and value.
