摘要
1目的探讨糖尿病性视网膜病变(diabetic retinopathy DR)的发病危险因素及其与血清配体CXC趋化因子配体16(CXC chemokine ligand16,CXCLl6)的关系。2方法按照1999年WHO糖尿病诊断标准,选取86例于我院内分泌科住院的2型糖尿病(T2DM)患者作为研究对象,其中伴有DR者作为观察组(DR组);单纯糖尿病者作为对照组(DM组)。所有受试者均测定CXCLl6、糖化血红蛋白(Hemoglobin A1c,HbA1c)及其他相关生化指标,并进行葡萄糖耐量试验(Oral Glucose Tolerance Test,OGTT)及胰岛素释放试验。此外测量全部受试者的身高、体质量、腰围、臀围以及血压。对两组患者血清CXCL16水平的差异及其与DR之间的关系进行分析。采用SPSS 17.0统计软件对数据进行分析处理。3结果 DR组的胰岛素抵抗指数(HMA-IR)、HbAlc、C反应蛋白(CRP)、空腹血糖、总胆固醇、甘油三酯、高密度脂蛋白胆固醇以及CXCL16水平明显高于DM组,差异有统计学意义,说明这些指标可能是DR的影响因素;血清CXCL16≥1.22μg/L的糖尿病患者发生DR的危险性是CXCL16<1.22μg/L的3.27倍;CXCL16是发生DR的主要危险因素。4结论 CXCL16是DR发生的危险因素之一。
Objective To explore the risk factors of diabetic retinopathy and its relationship with serum ligand CXCL16.Methods According to the 1999 WHO diagnostic criteria for diabetes,86 cases were selected as the research object in our hospital endocrinology hospitalized with type 2 diabetes （T2DM） patients, including those associated with diabetic retinopathy as the observation group（DR group） ;patients with diabetes as control group （DM group）. All subjects were measured CXCL16, HbAlc and biochemical indices,glucose tolerance test（Oral Glucose Tolerance Test, OGTT）and insulin releasing test.In addition all subjects were measured height, weight, waist circumference, hip circumference and blood pressure.Serum CXCL16 levels were analyzed in relationship and differences between the two groups. Using SPSS17.0 statistical software to analyzed the results process. Results HMAIR, HbA1c, CRP, fasting glucose, total c CL16 levels in DR group were signi indicating that these indicators may ficantly h be the inf holesterol, triglycerides, high density lipoprotein and CX- igher than DM group, there was significantly difference, luencing factors of DR.Serum CXCL16 occurs in diabetics 1.22μg/L or more diabetic retinopathy risk was CXCL16 in 1.22μg/L below 3.27 times.CXCL16 was a major risk factor for diabetic retinopathy.Conclusion CXCL16 is one of the risk factors of diabetic retinopathy
