肺泡表面活性物质联合容量目标通气治疗NRDS的疗效分析 被引量:7

摘要 目的:探讨肺泡表面活性物质(PS)联合容量目标通气(VTV)治疗新生儿呼吸窘迫综合征(NRDS)的疗效。方法:80例NRDS患儿按照随机数表法分为观察组、对照组,各40例。两组均接受常规治疗,观察组再加入PS联合VTV治疗。治疗24h后,评估两组患儿的治疗效果、动脉血气指标、血清Th1/Th2细胞因子含量。结果:两组治疗24h后,观察组治疗总有效率、基本痊愈率均高于对照组,无效率低于对照组(P<0.05);观察组的酸碱度、动脉血氧分压、动脉血氧分压/肺泡氧分压水平高于对照组,动脉血二氧化碳分水平低于对照组(P<0.05);观察组的血清Th1细胞因子干扰素-γ、白介素-1含量低于对照组,Th2细胞因子白介素-4、白介素-10含量高于对照组(P<0.05)。结论:PS联合VTV可有效提升NRDS治疗效果、优化肺通气换气功能,恢复Th1/Th2细胞因子稳态。 Objective: To investigate the effect of pulmonary surfactant(PS) combined with the capacity of the target ventilation(VTV) in treatment of neonatal respiratory distress syndrome(NRDS) effect. Methods: 80 cases of NRDS were randomly divided into observation group and control group, 40 cases in each. The two groups were treated with conventional therapy, and the observation group was treated with PS combined with VTV. Treatment effect, arterial blood gas index, serum Th1/Th2 cytokine levels were assessed after treatment with 24 h. Results: After the treatment of two groups of 24 h, the total efficiency of the observation group, the basic recovery rate was higher than the control group, no efficiency is lower than that of the control group(P〈0.05); the observation group p H, Pa O2 and Pa O2/Pa O2 level higher than the control group, arterial blood carbon dioxide levels lower than the control group(P〈0.05); observation group serum Th1 cytokine interferon gamma, interleukin-1 levels lower than the control group, Th2 cytokine interleukin-4 and interleukin-10 levels were higher than the control group(P〈0.05). Conclusion: PS combined with VTV can effectively improve the therapeutic effect of NRDS, optimize the ventilation function of lung ventilation, and restore the stability of Th1/Th2 cytokines.
