摘要
目的探讨全麻术中预注射磷酸肌酸钠对患者术后躁动及恢复的影响。方法选取2014年1月~2015年7月杭州市肿瘤医院符合纳入标准且择期腹部手术需要全身麻醉患者52例,按随机数字表法分为研究组和对照组,各26例,研究组麻醉诱导后给予静脉输注磷酸肌酸钠1.0 g,对照组给予静脉输注等容量的生理盐水,测定术后躁动评分及镇静评分,同时对比2组手术不同时间点认知功能及血清炎症因子、免疫球蛋白、免疫细胞亚群等指标。结果与治疗前比较,2组手术后T_2、T3时MMSE和WMS评分降低,血清IgA、IgG、IgM及CD3^+、CD4^+、CD4^+/CD8^+含量降低,手术后T_2、T_3、T4时血清IL-2、IL-8、TNF-α升高,差异均具有统计学意义(P<0.05);与对照组比较,研究组手术后T_2、T_3时MMSE和WMS评分较高,手术后T_3、T_4时血清IgA、IgG、IgM含量较高,手术后T_2、T3、T4时血清CD3^+、CD4^+、CD4^+/CD8^+较高,手术后T_2、T_3、T_4时血清IL-2、IL-8、TNF-α较低,差异均具有统计学意义(P<0.05);2组间在拔管时间、手术时间及睁眼时间上无差异;对照组躁动评分高于研究组,镇静评分低于研究组,差异具有统计学意义(P<0.05)。结论全麻术中预注射磷酸肌酸钠能抑制苏醒期躁动发生,抑制拔管反应,提高术后认知功能及免疫功能,效果显著。
Objective To investigate the effect of pre-injection of creatine phosphate on restlessness and recovery after general anesthesia.Methods 52 cases in our hospital who were met the inclusion criteria for elective abdominal surgery requires anesthesia were randomly divided into study group and control group,26 cases in each group,the study group received sodium phosphocreatine 1. 0g pre injection after anesthesia induction,and the control group received normal saline infusion volume,postoperative agitation score and sedation score were determined,and compared the cognitive function and serum inflammatory factor,immune globulin,immune cell subsets and other indicators after operation. Results Compared with before treatment,MMSE and WMS score,serum IgA,IgG,IgM,CD3^＋,CD4^＋and CD4^＋/ CD8^＋decreased in 2 groups at T2 and T3,serum IL-8 and serum IL-2,TNF-α increased at T2,T3,and T4,the difference was statistically significant（ P〈0. 05）. Compared with the control group,MMSE and WMS score in the study group were higher at T2 and T3,serum IgA,IgG,IgM were higher at T3 and T4,serum CD3^＋,CD4^＋,CD4^＋/ CD8^＋were higher at T2,T3 and T4,serum IL-8 and serum IL-2,TNF-α were lower at T2,T3 and T4,the difference was statistically significant（ P〈0. 05）. And tube time,operation time and the time of opening eyes in pull between the two groups had no difference. compared with the control group,agitation in the study group was lower,sedation score were higher,the difference was statistically significant（ P〈0. 05）. Conclusion Creatine phosphate sodium in pre injection in general anesthesia was significant effect,it can inhibit restlessness,inhibit extubation,improve the postoperative cognitive function and immune function.
