人血丙种球蛋白对小儿难治性支原体肺炎的实效性评价 被引量:9

摘要 目的观察并探讨人血丙种球蛋白(intravenous immune globulin,IVIG)对小儿难治性肺炎支原体肺炎(refractory mycoplasma pneumonia,RMPP)的有效性和安全性。方法选取2014年3月-2015年7月期间在杭州市第一人民医院儿科确诊RMPP并接受治疗的患儿84例,随机为对照组和观察组各42例。对照组给予RMPP常规治疗(包括抗感染、止咳、吸氧等);观察组在常规治疗基础上加用IVIG治疗。统计比较2组咳嗽减轻时间、喘息减轻时间、体温恢复时间和肺部罗音消失时间,并在治疗前后检测2组补体3(C3)、补体4(C4)、IgG水平,并评价综合疗效。结果观察组咳嗽减轻时间、喘息减轻时间、体温恢复时间和肺部罗音消失时间分比为(5.0±0.6)、(3.3±0.4)、(2.8±0.5)和(4.8±0.6)d,对照组为(6.7±0.7)、(3.9±0.5)、(3.7±0.7)和(5.6±0.5)d,观察组各项症状改善时间明显短于对照组(P〈0.05);治疗后,观察组IgG水平明显提升,且明显高于对照组(P〈0.05);观察组有效率为95.2%(40/42),对照组为76.2%(32/42),观察组有效率明显高于对照组(P〈0.05)。结论 IVIG治疗RMPP能明显改善患儿的临床症状,提升免疫力,疗效确切。 Objective To observe and explore the effect and safety of intravenous immunogloblin( IVIG) in the treatment of refractory mycoplasma pneumonia( RMPP). Methods 84 cases received treatment in the hospital from May 2014 to July 2015 were randomly divided into the control group and the observation group with 42 cases in each group. The control group were given RMPP conventional therapy( including antibiotics,cough,oxygen,etc.); observation group were treated with IVIG treatment on the basis of conventional therapy. Cough relief time,wheezing alleviate time,body temperature recovery time and pulmonary rales disappeared time were recorded and complement 3( C3),complement 4( C4),IgG levels were detected before and after treatment,comprehensive efficacy was evaluated. Results Cough relief time,wheezing alleviate time,body temperature recovery time and pulmonary rales disappeared time in observation group were( 5. 0 ± 0. 6) d,( 3. 3 ± 0. 4) d,( 2. 8 ± 0. 5) d and( 4. 8 ± 0. 6) d,while in the control group were( 6. 7 ± 0. 7) d,( 3. 9 ± 0. 5) d,( 3. 7 ± 0. 7) d and( 5. 6 ± 0. 5) d,the symptoms improvement time in observation group were significantly shorter than the control group( P〈0. 05); after treatment,IgG levels were significantly improved in the observation group,and was significantly higher than in the control group( P〈0. 05); the efficiency of observation group was 95. 2%( 40 /42),while in the control group was76. 2%( 32 /42),the observation group was significantly higher( P〈0. 05). Conclusion IVIG treatment of RMPP can significantly improve the clinical symptoms of children,enhance the immune system,with exact effect.
