摘要
目的分析炎性肌纤维母细胞瘤(IMT)的影像表现,提高对该病的认识。方法回顾性分析11例经手术病理证实的IMT患者的临床、影像和病理资料,分析其影像表现特点及其与病理特征的联系。11例中9例行CT扫描(6例增强扫描),1例行CT增强扫描和MRI平扫,1例仅行MRI增强检查。结果 11例肿瘤均为单发,其中颈部2例,肺部2例,脾3例,肾、胃体、睾丸和下肢各1例;CT平扫表现为囊实性肿块3例,实性肿块7例。7例增强扫描呈均匀或不均匀轻中度到明显强化,3例呈环形渐进性强化,2例呈斑片状强化,1例均匀强化,1例无明确肿块,增强扫描见多发条形血管影;MRI T1WI以低信号为主混杂信号,T2WI为混杂略高信号;边界清晰。其中4例内见条形、结节状钙化,2例病灶周围见渗出。结论 IMT临床和影像学表现缺乏特异性,诊断困难,但肿瘤内钙化、瘤周渗出和环形渐进性强化方式可提示该病的诊断。
Objective To describe the radiology features of inflammatory myofibroblastic tumor （IMT）. Methods The clinical and pathology materials findings Radiology（ n = 11 ）, of pathologically confirmed IMT were retrospectively analyzed. CT scan was performed in 10 patients including 7 dynamic enhanced CT. MRI examination was undertaken in 2 patients including 1 enhanced scan. Results Plain CT scan was performed in 9 patients （ including 6 enhanced scan）, MRI and CT enhanced scan in 1 patient and MRI enhanced scan in 1 patient. All of the tumors were single solid lesions lo- cated in the neck （n = 2）, lung（ n = 2）, spleen （n = 3 ）, kidney, stomach, testicle and lower limbs （n = 1 ）. Of the 10 pa- tients, 3 were seen as a cystic or solid mass and 7 were solid masses. After contrast administration, the lesions enhancement was mild to significant. 3 masses enhanced progressively from the periphery to the center, 2 cases showed nodular enhancement, 1 case revealed homogeneous enhancement and 1 case in the testicle showed no tangible tumor with strip enhanced vessels. On MRI, the masses revealed mixed signals of heterogeneous isointense and hypointense on T1 - weighted images and T2 - weighted images with well or ill - defined boundaries. Of all the patients, 4 tumors revealed punctate or stippled calcification and 2 revealed surrounding infiltration. Conclusion The imaging features of IMT may be nonspecific; however, some imaging features, such as calcification, infiltration, peripheral enhancement, might be useful in diagnosing IMT. Radiological features can accurately present the location, boundary, extent of involvement, internal structure, etc; this can provide useful information for the diagnosis and differential diagnoses of the IMT.
