摘要
目的探讨大咯血介入栓塞治疗时应用支气管动脉CT血管造影(CTA)的方法与效果。方法对大咯血的84例患者行介入栓塞治疗,且于治疗前行支气管动脉CTA检查,治疗时再次明确患者的出血动脉,并均行介入栓塞治疗。治疗1周后再行支气管动脉CTA复查。回顾性分析支气管动脉CTA在该疾病患者治疗中的应用价值。结果所有大咯血患者治疗前进行支气管动脉CTA检查,共发现出血的病变血管112支,介入栓塞治疗时共发现出血的病变血管共123支,支气管动脉CTA检查的诊断符合率为91.06%。患者支气管动脉CTA检查后均行介入栓塞治疗且治疗均成功,介入栓塞治疗后共有76例患者回院进行支气管动脉CTA检查,检查结果表明患者病变血管栓塞治疗后均获良好栓塞效果。结论术前支气管动脉CTA检查能够为大咯血患者的介入栓塞治疗提供有效的诊断依据,术后再行该检查则可有效评估患者的疗效。
Objective To investigate the method and effect of bronchial artery CT angiography （CTA） in the treatment of massive hemoptysis by interventional embolization. Methods 84 patients with massive hemoptysis underwent interventional embolization and treatment before bronchial artery CTA examination were selected, treated once again clear in patients with arterial hemorrhage and underwent interventional embolization. After 1 week treatment, the bronchial artery CTA was reviewed. We retro-spectively analysed the application value of bronchial artery CTA in the treatment of patients with the disease. Results Massive hemoptysis patients before treatment, all of bronchial artery CT angiography （CTA）, were found in 1 t2 branches of the bleeding vessel, interventional embolization were found in the bleeding vessel 123 branch, in the diagnosis of bronchial artery CT angiogra- phy （ CTA）, the rate was 91.06%. Patients with bronchial artery CTA were performed after interventional embolization and treatment were successful, after interventional embolization in the treatment of a total of 76 cases of patients with back to school for bronchial artery CTA examination, examination results showed that vascular lesions in patients with embolization after all obtained good effect of embolization. Conclusion Preoperative bronchial artery CTA examination can provide an effective diagnostic basis for interventional emholization in the treatment of massive hemoptysis.
