摘要
目的:探讨窝沟封闭术联合氟保护漆在龋齿患儿中的预防效果。方法:回顾性分析30例龋齿患儿的临床资料,按治疗方法的不同将陕西省榆林市第一医院口腔科收治的30例龋齿患儿分为窝沟封闭术治疗组(A组)、含氟保护漆治疗组(B组)和窝沟封闭术联合含氟保护漆治疗组(C组),每组10例,观察并随访比较3组患儿治疗6个月后、治疗12个月后及治疗24个月后龋齿发生率和封闭剂脱落情况。结果:所有患儿治疗6个月后均无龋齿患儿出现,治疗12个月后A组2例患儿发生龋齿,B组3例发生龋齿,C组无龋齿发生,3组比较差异无统计学意义(P>0.05);治疗24个月后A组、B组均有6例患儿发生龋齿,龋齿发生率均为60.00%,C组只有1例龋齿患儿,龋齿率为10.0%,C组分别与A组、B组比较差异有统计学意义(P<0.05),A组与B组龋齿发生率比较差异无统计学意义(P>0.05)。治疗6个月后,A组患儿封闭剂均保持完整,无脱落情况;B组有1例患儿含氟保护漆发生部分脱落,总脱落率为10.0%;C组患儿封闭剂保持完整,无脱落情况发生,3组患儿封闭剂脱落情况比较差异无统计学意义(P>0.05)。治疗12个月后,A组2例患儿封闭剂有部分脱落现象,总脱落率为20.0%;B组7例患儿保持完整,总脱落率为30.0%;C组无一例患儿封闭剂脱落,均保持完整状态,3组儿童封闭剂脱落情况比较差异无统计学意义(P>0.05)。治疗24个月后,A组、B组均有4例患儿封闭剂保持完整,总脱落率均为60.0%;C组9例保持完整,总脱落率为10.0%,C组脱落率与A组、B组比较差异有统计学意义(P<0.05)。结论:窝沟封闭术联合含氟保护漆能够有效预防儿童龋齿,且具有较高安全性,操作简单方便,儿童易于接受,值得临床推广应用。
Objective：To study the protective effect of the combination of fissure sealants and fluoride protector on children with dental caries.Methods：30cases of children with dental caries from June 2012 to January 2014 were recruited for retrospective analysis.The children were divided into the pit and fissure sealant group（group A）,the fluoride-containing protection group（group B）,and the pit and fissure sealant combined fluoride-containing protection group（group C）.10 cases for each group.All cases were observed and recorded for the incidence of caries and the sealants shedding after 6,12 and 24months of treatment.Results：No children got dental caries after 6months of treatment.2cases in group A and 3cases in group B were found having caries,but no dental caries was found in group C after 12 months of treatment.There was no significant difference among 3groups（P〉0.05）.24 months after treatment,there were 6cases（60%）of children＇s dental caries in group A and B respectively,while there was only one case（10%）of dental caries in group C（P〈0.05,group C vs.group A or group B）.There was no significant difference among 3groups with regard to the incidence of sealants shedding after 6and 12 months of teatment（P〉0.05）.However,24 months after treatment,the incidence of sealants shedding in group C showed significant difference（P〈0.05）.Conclusion：The combination of fissure sealants and fluoride protector can achieve the best effect protecting children from dental caries,with high safety and simple operation.Therefore,this technique is easy to be accepted by children and is widely recommended for clinical practice.
