分层应变技术评价糖耐量异常患者左心室各层心肌收缩功能改变 被引量:1

Evaluation of the left ventricular systolic function of patients with impaired glucose tolerance by layer-specific strain

摘要 目的应用分层应变技术探讨糖耐量异常患者左心室各层心肌收缩功能的改变。方法对40例糖耐量异常患者和36例正常对照者进行常规超声心动图测量,存储二维动态图像后在EchoPAC工作站进行描记分析,获得左室壁18个节段内层、中层及外层心肌的收缩期圆周应变峰值(circumferential strain, CS)、收缩期纵向应变峰值(longitudinal strain,LPS)及整体的分层和全层应变值。结果糖耐量异常组患者整体的内层心肌、中层心肌及整体全层心肌CS低于正常对照组(P〈0.05),整体外层心肌的CS、整体各层及整体全层LPS与正常对照组比较差异无统计学意义(P〉0.05)。基底段、中间段的内层、中层心肌CS和LPS均低于正常对照组(P〈0.05);基底段、中间段外层心肌及心尖段的各层心肌CS、LPS与正常对照组比较差异无统计学意义(P〉0.05)。结论分层应变技术可定量评价糖耐量异常患者左心室各层及整体心肌的周向和纵向收缩功能,对于评价糖耐量异常患者左心室早期心肌收缩功能的改变具有一定的临床应用价值。 Objective To explore the systolic function of the left ventricular of patients with impaired glucose tolerance by layer-specific strain. Methods The conventional echocardiography was performed in 40 patients with impaired glucose tolerance and 36 normal control subjects by application of GE Vivid E9. A tracing analysis was conducted on the off-line EchoPAC software after storing the two-dimensional dynamic images. The peak systolic circumferential strain(CS) and peak systolic longitudinal strain(LPS) of whole stratification and full-thickness were obtained from inner, medial and outer myocardial (subendocardial, mid- myocardial and epicardial) layers of 18 segments in the left ventricular walls. Results The statistical significance was observed in the CS between the whole inner, medial myocardial layers and full-thickness in the left ventricles of two groups ( P d0.05). No statistical difference was revealed in the CS of the whole outer myocardial layer and in the LPS of the whole inner, medial and outer myocardial layers and full- thickness in the left ventricles of the two groups ( P〉0.05). The statistical significance was found in the CS and LPS of inner, medial myocardial layers of basal and middle segments between the two groups ( P〈0.05). No difference was detected in the CS and LPS of three myocardial layers of apical segments and inner, medialmyocardial layers of basal and middle segments between the two groups (P〉0.05 ). Conclusions Layer-specific strain can quantitatively evaluate the changes of systolic function on all myocardial layers and full-thickness of left ventricles in patients with impaired glucose tolerance, which shows a certain clinical application value in assessing early functional changes of left ventricles in the patients with impaired glucose tolerance.
