摘要
目的通过疾病诊断相关组(Diagnosis related groups,DRGs)干预系统综述,探索适合我国国情的疾病分组内涵、应用及发展。方法检索中外文数据库发表的有关基于"疾病诊断相关分组"文章,对DRGs的研究影响进行系统综述和meta分析。结果入选中文文献69篇,英语文献258篇,DRGs主要包括医疗服务管理,费用管理和绩效管理3个方面内涵。meta分析结果显示:与对照组相比,DRG组医保参与率,住院天数,药占比,手术费用比,并发症和满意度等医疗服务、费用及绩效管理,均呈明显优势。结论 DRGs被认为是解决全球医疗管理领域应用广泛的一种病例组合系统,通过文献的系统综述,对更加全面准确地评价影响DRGs的风险因素具有重要意义。
Objective To identify that had been proposed, described, or used regardless of their initial objective and to explore and develop the diagnosis related groups as prospective payment system （DRGs） in China. Methods The literatures about the effect of the diagnosis related groups based payment systems were retrieved from CNKI、WF 、PubMed、Embase、Cochrane Library. All the related literatures were checked and qualified in the concept on this adjustment factor relationship. Results All literatures were included. Meta analysis indicated that statistical differences existed in the medical service、performance management and healthcare cost by DRGs independent practice. Conclusion Large improvements in statistical rules can be achieved with the use of DRGs to characterize baseline case mixes. These improvements are important to identify DRGs related risk factors .
