摘要
目的 探讨不同牙体预备方法对重度楔状缺损牙体桩核冠修复后抗折特性的影响,为临床缺损牙体桩核冠修复治疗提供可靠依据。方法 随机选取2015年1月~2016年2月来我院口腔科就诊并行正畸治疗而拔除新鲜下颌第一前磨牙患者56例(56颗),按不同牙体预备方法及桩核冠修复方法分为A、B、C、D、E、F、G、H组,共8组,每组7颗,建立重度楔状缺损牙模型后,测试各组经桩核冠修复后的抗折性能。结果 A组抗折载荷高于E组(P〈0.05),B组抗折载荷高于F组(P〈0.05),F组抗折载荷高于E组(P〈0.05),G组抗折载荷高于H组(P〈0.05),其中B组、F组可修复折裂比例均为42.86%,而其余各组可修复折裂比例均为0。结论 对重度楔状缺损牙体进行桩核冠修复时,保留颊侧悬突牙体组织可提高其抗折性能,桩核冠修复时不建议制备牙本质肩领,同时采取纤维桩核修复能促进牙体折裂后的再修复。
Objective To investigate the influence of different tooth preparation methods for anti fracture characteristics of severe wedge shaped defect of post and core restorations, and to provide a reliable basis for the clinical treatment of defect tooth post and core crown. Methods 56 patients (56 teeth) were extracted from fresh mandibular first molar and who treated with concurrent orthodontic treatment in our hospital department of stomatology from January 2015 to February 2016 were randomly selected, and according to the different tooth preparation method and the post core crown restoration method were divided into A, B, C, D, E, F, G, H group, there were 8 groups, 7 teeth in each group.Then, the fracture resistance of each group were tested after the post core crown restoration. Results The bending load of group A was higher than that of group E(P〈0.05); The bending load of group B was higher than that of group F(P〈0.05); The bending load of group F was higher than that of E group(P〈0.05); The bending load of group G was higher than that of group H(P〈0.05); Among them, the percentage of the repaired breaks in the group B and the group F were 42.86%, and the percentage of fracture in the other groups was O. Conclusion The repair of post core crown with severe wedge shaped defect, retaining the buccal suspension can improve the fracture resistance, and it is not recommended to pre- pare the shoulder collar in the post core crown restoration, at the same time, the restoration of the fiber post and core can promote the restoration of the fractured teeth.
