摘要
目的:通过调查分析韶关地区儿童再发性腹痛(RAP)与幽门螺杆菌感染(Hp)的相关性,以为疾病临床治疗提供依据。方法:以我院2014年1月~2015年5月诊治的120例再发性腹痛患儿为研究对象,均通过13C尿素呼气试验、胃镜检测Hp阳性情况,统计Hp感染率、Hp感染与消化道疾病的关系,且通过单因素、多因素Logistic回归分析RAP患儿Hp感染影响因素。结果:120例患儿中Hp感染45例,其中慢性浅表性胃炎Hp感染8例,消化道溃疡Hp感染7例;Hp感染与胃黏膜病变程度有关(Z=25.126, P<0.01)。对患儿年龄、性别等临床资料行单因素、多因素Logistic回归分析:患儿年龄、喂食方式、胃病家族史、消化道溃疡为再发性腹痛患儿Hp感染独立影响因素。抗Hp治疗后45例Hp感染患儿腹痛缓解率77.8%。结论:Hp感染是导致RAP发病的主要原因之一,且Hp感染与患儿年龄、喂食方式、消化道溃疡、胃病家族史有关,给予Hp感染防治至关重要。
Objective:To investigate the correlation between recurrent abdominal pain (RAP ) and helicobacter pylori (Hp)infection in children from Shaoguan area,and to provide evidence for clinical treatment of the disease. Methods:A total of 120 children with RAP,who were hospitalized in our hospital between January 2014 and May 2015,were included as the subjects in the study. All patients underwent 13C urea breath test and gastroscopy to determine Hp positive rate and statistically analyze the correlation between Hp infection rate,Hp infection and gastrointestinal disease. The factors of Hp infection in RAP children were determined by univariate and multivariate Logistic regression analysis. Results:There were 45 cases of Hp infection in 120 children,including 8 of chronic superficial gastritis Hp infection and 7 of peptic ulcer Hp infection. Hp infection was correlated with the degrees of gastric mucosal lesions (Z=25.126,P〈0.01). The clinical data of children′s age and gender were determined by univariate and multivariate Logistic regression analysis. Children′s age, feeding habit,family history of gastropathy,and peptic ulcer were independent factors of Hp infection in RAP children. After the anti-Hp treatment,the remission rate of abdominal pain in 45 cases of Hp infection was 77.8%.Conclusion Hp infection is one of the major causes of RAP,correlated with children′s age,feeding habit,peptic ulcer,and family history of gastropathy. The prevention and treatment for Hp infection is significant.
