良恶性卵巢肿瘤的多普勒超声特征及Alaczar超声评分鉴别价值 被引量:7

Doppler Ultrasonographic Features and Alaczar Ultrasound Score in the Differential Diagnosis of Benign and Malignant Ovarian Tumors

摘要 目的探讨超声形态学特征、血流动力学特征及Alaczar超声评分对良恶性卵巢肿瘤的鉴别诊断价值。方法选取2013年11月至2015年11月大连市妇幼保健院妇科诊治的卵巢原发性肿瘤患者170例,其中卵巢恶性肿瘤85例,卵巢良性肿瘤85例。观察良恶性卵巢肿瘤的超声声像图特征,比较良恶性卵巢肿瘤Alaczar超声评分、血流阻力指数(RI)及中心血流的差异。以病理学结果为金标准,观察Alaczar超声评分对恶性卵巢肿瘤的诊断效力。结果卵巢恶性肿瘤Alaczar超声评分显著高于卵巢良性肿瘤[(6.27±1.91)分比(3.56±1.02)分],RI显著低于卵巢良性肿瘤[(0.42±0.12)分比(0.59±0.16)分],其存在中心血流的比例显著高于卵巢良性肿瘤[54.1%(46/85)比30.6%(26/85)],差异有统计学意义(P〈0.05)。临床分期Ⅲ~Ⅳ期临床恶性肿瘤Alaczar超声评分显著高于临床分期Ⅰ~Ⅱ期者[(5.91±1.85)分比(6.78±1.98)分],差异有统计学意义(P〈0.05)。以手术病理及穿刺细胞学结果为金标准,Alaczar超声评分对卵巢恶性肿瘤诊断灵敏度、特异度、阳性预测率、阴性预测率及准确度分别为90.6%、88.2%、88.5%、90.4%和89.4%,均高于RI(71.8%、52.9%、60.4%、65.2%、62.4%)及中心血流(83.5%、64.7%、70.3%、79.7%、74.1%)。结论超声形态学特征、血流动力学特征及Alaczar超声评分可以为良恶性卵巢肿瘤鉴别诊断提供有价值的信息,其中Alaczar超声评分价值最高。 Objective To investigate the diagnostic value of ultrasound in differential diagnosis of benign and malignant ovarian tumors by ultrasonic morphology,hemodynamics and Alaczar. Methods From Nov. 2013 to Nov. 2015,170 cases of ovarian cancer treated in Dalian City Maternal and Child Health Hospital were selected as the research objects,85 cases of malignant tumor,85 cases of benign tumors. Ultrasonographic characteristics,Alaczar ultrasound score,RI and central blood flow of benign and malignant ovarian tumors were observed and compared. With the result of pathology as the gold standard,the efficacy of Alaczar in diagnosis of malignant ovarian tumors was observed. Results The Alaczar score of malignant ovarian tumor was significantly higher than the benign ovarian tumor [( 6. 27 ± 1. 91) score vs( 3. 56 ± 1. 02)score],RI was significantly lower than the benign ovarian tumors[( 0. 42 ± 0. 12) score vs( 0. 59 ± 0. 16)score],the proportion of blood flow in the central blood flow was significantly higher than the benign ovarian tumors[54. 1%( 46 /85) vs 30. 6%( 26 /85) ],the differences were statistically significant( P〈0. 05). The Alaczar ultrasound score of clinical stage Ⅲ-Ⅳ malignant tumor was significantly higher than the clinical stageⅠ-Ⅱ[( 5. 91 ± 1. 85) score vs( 6. 78 ± 1. 98) score],the difference was statistically significant( P〈0. 05). The diagnostic sensitivity,specificity,positive predictive rate,positive predictive rate and accuracy of Alaczar ultrasound score in malignant ovarian tumors were 89. 4%,88. 2%,88. 5%,90. 4% and 90. 6%respectively,all higher than those of RI( 71. 8%,52. 9%,60. 4%,65. 2%,62. 4%) and central blood flow( 83. 5%,64. 7%,70. 3%,79. 7%,74. 1%). Conclusion Ultrasonic morphology,hemodynamic characteristics and Alaczar score can provide valuable information for the differential diagnosis of benign and malignant ovarian tumors,among which,the value of Alaczar is the highest.
