摘要
目的通过查阅长春西汀产品说明书内容、不良反应以及配伍禁忌,评价长春西汀的安全性,提出风险管理建议。方法对比国内外长春西汀药品说明书,检索《中国期刊全文数据库》获得有关长春西汀不良反应及配伍禁忌的文献,进行汇总分析。结果国内不同厂家不同剂型长春西汀的说明书存在一定差异,主要是在用法用量、不良反应、注意事项及药物相互作用等方面。检索长春西汀不良反应文献31篇,其中循环系统的AIDR发生率居首位,占33.3%,其次为精神神经系统,占25.9%。检索长春西汀配伍禁忌文献27篇,其与多种药物存在配伍禁忌,且多发生于两种输液续滴过程,比例高达39.53%。注射用长春西汀的溶剂宜选择0.9%氯化钠注射液或5%葡萄糖注射液,在溶剂体积方面,宜选用500 mL溶剂。结论不同专业背景的医务人员对长春西汀安全性和风险防范措施的理解存在差异,有必要向医务人员开展针对性的用药宣传教育,从而保证患者的用药安全。同时建议以原研药品说明书提供的信息为参照,掌握此类药物的安全用药信息,为临床提供完善的药学服务。
Objective By reading through vinpocetine instruction,adverse reactions and incompatibility,the safety of vinpocetine was evaluated to give the risk management suggestions.Methods Compare vinpocetine instructions and retrieve the CNKI to get literatures about adverse reactions and incompatibility for Meta-analysis.Results There were slight differences in dosage and administration,adverse reaction,precautions and drug interactions in vinpocetine instructions among different manufacturers.Totally 31 related ADR reports on vinpocetine were searched,and in these reports,the incidence of circulatory system ADR ranked the first,accounting for 33.3%,and the second was the nervous system and mind,accounting for 25.9%.Totally 27 related incompatibility reports on vinpocetine were searched,we found vinpocetine had incompatibility with many drugs,which occurred when one intravenous dripping was followed by another,accounting for 39.53%.Ggenerally 0.9%sodium chloride or 5%glucose injection was chosen as solvent of vinpecetine,and the volume of solvent generally was 500 mL.Conclusion Different professional backgrounds of the medical staff have different understanding on safety and risk prevention measures of vinpocetine,so it is necessary for the medical staff to develop drug education to ensure the patient's safety.We suggest the medical staff should grasp the information of drug safety with the drug instruction as reference in order to provide perfect clinical pharmaceutical services.
