摘要
目的研究及观察急性盆腔脓肿手术抗感染治疗患者腹腔液中的微生物菌群分布及炎性因子水平变化情况,为本病的治疗提供依据。方法选取2013年10月-2015年12月医院收治50例急性盆腔脓肿患者为研究对象,将其随机分为对照组和观察组,各25例;对照组进行术前常规治疗,观察组则在术前进行7d抗感染治疗,比较两组患者治疗前和术后3d、5d的腹腔液微生物菌群情况及抗炎因子、促炎因子水平进行比较,数据采用SPSS 15.0软件进行统计分析。结果观察组治疗前及术后3d、5d总菌株检出率分别为88.00%、24.00%及4.00%,术后3d及5d观察组均低于对照组,且两组中均以革兰阴性菌为主;腹腔液抗炎因子及促炎因子水平均优于对照组,差异均有统计学意义(P<0.05)。结论急性盆腔脓肿手术术前抗感染治疗对患者腹腔液中的微生物菌群及炎性因子水平的影响均更为积极,因此临床应用价值相对更高。
OBJECTIVE To study and observe the changes of peritoneal fluid microbial flora distribution and inflammatory factors levels of patients with anti-infection treatment of acute pelvic abscess surgery,in order to provide evidence for the treatment of disease.METHODS A total of 50 patients with acute pelvic abscess in hospital from Oct.2013 to Dec.2015 were selected as the research objects,and were randomly divided into control group and observation group,with 25 cases in each group.The patients in control group were treated with conventional preoperative treatment,and in observation group were treated with anti-infection treatment for 7days.Then the peritoneal fluid microbial flora situation and anti-inflammator,pro-inflammatory factors of the two groups before the treatment and at third and fifth day after the surgery were compared.The data were analyzed by SPSS 15.0.RESULTS The total bacterial strains rates of observation group before the treatment and at third and fifth day after the surgery were 88.00%,24.00% and 4.00%,respectively,and the rates at third and fifth day after the surgery were lower than those of control group.The gram-negative bacteria were predominant in both groups.The peritoneal fluid anti-inflammator and pro-inflammatory factors of observation group were all better than those of control group,the differences were all significant（P〈0.05）.CONCLUSION The influence of preoperative anti-infection treatment for the peritoneal fluid microbial flora distribution and inflammatory factors levels of patients with acute pelvic abscess surgery are more active,so its clinical application value is relatively higher.
