摘要
目的对比观察内外侧钢板固定植骨结合经皮电刺激与髓内针植骨治疗肱骨远端骨不连接的治疗效果及对关节功能的影响。方法 80例肱骨远端骨不连患者根据数字表法随机分为观察组和对照组,每组40例。观察组采用内外侧钢板固定植骨结合经皮电刺激的治疗方式,对照组采用髓内针植骨的治疗方式。对比观察两组的临床治疗效果。结果两组患者的骨不连接均愈合良好,观察组患者的手术时间[(103.85±25.97)min]、术中出血量[(76.41±19.11)ml]和骨折愈合时间[(2.57±0.65)个月]明显少于对照组[(156.68±39.15)min,(202.75±50.69)ml,(3.74±0.94)个月],差异有统计学意义(P<0.05);两组患者的住院时间、NEER评分和HSS评分无显著的差异(P>0.05);两组患者术后均没有发生严重的并发症。结论内外侧钢板固定植骨结合经皮电刺激和髓内针植骨的两种手术方法均能明显改善肱骨远端骨不连患者的肩关节和肘关节功能,但采用内外侧钢板固定植骨结合经皮电刺激的手术时间较短、术中出血量较少、骨折愈合较快,效果优于髓内针植骨治疗。
Objective To investigate the effect of internal and external side plate fixation bone combined with transcutaneous electrical nerve stimulation compared with the intramedullary nail bone grafting treatment of distal humeral nonunion and joint function.Methods 80 cases of distal humerus bone nonunion in our hospital,which were randomly divided into the observation group and the control group,40 cases in each group.The observation group were given the lateral plate fixation and bone graft treatment,and the control group were given intramedullary pin bone graft treatment,and the clinical efficacy of two groups of patients were compared.Results Two groups of patients with bone nonunion were healed,the operative time(103.85±25.97) min,the amount of intraoperative bleeding(76.41±19.11) ml and fracture healing time(2.57±0.65) months were significantly less than those of the control group(156.68±39.15) min,(202.75±50.69) ml,(3.74±0.94) months,and the difference were statistically significant(P〈0.05);there was no significant difference between two groups of patients in hospitalization time,NEER score and HSS score(P〉0.05);and two group patients didn't appeared serious complications.Conclusion Two surgical methods can significantly improve the bone nonunion in distal humerus in patients with shoulder joint and elbow joint function,but the operation time of internal and external side plate fixation bone combined with transcutaneous electrical nerve stimulation in treatment of distal humeral nonunion is short with less operation bleeding and faster fracture healing.Therefore,it is better than that of spinal cord inner pin bone grafting.
