摘要
糖尿病患者创面难愈是由体内高糖环境和多种生物学因素共同作用导致的疾病。患者皮肤因局部微环境改变及免疫趋化应答功能障碍,容易受损,且创面反复溃烂,难以愈合,最终形成顽固性难愈创面,临床尚缺乏切实有效的治疗方法。对于严重下肢溃疡如糖尿病足的患者,临床多行截肢术阻止创面蔓延,降低患者生活质量,危害患者的生命安全。近年来,随干细胞技术、分子生物学技术、基因技术的不断发展,一些新的治疗方法如利用羊膜搭载表皮干细胞构建组织工程皮肤等被相继应用于治疗创面难愈的实验室和早期临床研究,取得较为满意的疗效。本文从糖尿病患者创面难愈机制,最新临床治疗方法及临床应用探索等方面进行综述。
Diabetic non-healing wound is a diabetic Complication resulting from hyper glucose environment in vivo and many biological factors. The skin of such patient is easily damaged, and forms deteriorating chronic skin ulcer because of the local environmental changes and damage of immune responses. Diabetic non-healing wound, as a complicated pathophysiologic phenomenon, lacks effective therapeutic methods. Patients with serious ulcer in lower limb such as diabetic foot, will undergo amputation,which again seriously threatens the life and life quality of diabetic patients. Recently, with the developments of stem cell technology, gene therapy and other new biological techniques, new treatments have been applied to laboratorial animal models and early clinical research. These academic studies have gained significant progress for the treatment of the diabetic non-healing wound. This article reviews the physiopathologic mechanisms of diabetic non-healing wound, latest therapeutic methods and associated clinical researches.
