摘要
目的探究连续股神经阻滞对兔膝骨性关节炎(KOA)模型的疼痛治疗效果及可能的作用机制。方法建立KOA兔模型。观察空白组、模型组和实验组(A、B、C、D和E组)动物行为学的变化,比较疼痛反应,然后使用ELISA法检测各组兔滑膜液中TNF-α、IL-1和IL-6的表达,HE染色观察KOA兔滑膜软骨组织形态学变化,最后使用免疫组化的方法测定滑膜和软骨组织中基质金属蛋白酶-3(MMP-3)及基质金属蛋白酶抑制剂-1(TIMP-1)的表达。结果相比于模型组,连续股神经阻滞能显著缓解KOA兔的疼痛(P<0.05或P<0.01);滑膜软骨组织损伤及周围炎性反应明显减轻;实验组兔滑膜液中TNF-α、IL-1和IL-6的表达也显著下降(P<0.05或P<0.01),其中C组、D组和E组更为明显(P<0.01);另外,连续骨神经阻滞处理使KOA兔滑膜和软骨组织中MMP-3和TIMP-1的表达显著下调(P<0.05或P<0.01),改善了MMP-3/TIMP-1的失衡。结论连续骨神经阻滞可以显著缓解KOA兔的疼痛反应,改善滑膜软骨损伤,其机制可能与下调滑膜液中TNF-α、IL-1和IL-6的表达以及改善滑膜和软骨中MMP-3、TIMP-1的异常表达有关。
Objective To explore therapeutic effects and mechanisms of continuous femoral nerve block for rabbit knee osteoarthritis (KOA) model. Methods KOA rabbit model was established. The changes of animal behavior were observed in control group, model group, and experimental groups (A, B, C, D and E group), and then pain responses were compared. Meanwhile, the expressions of TNF-a, IL-1 and IL-6 were detected by ELISA. The morphological changes of synovial cartilage tissue were identified by HE staining, and finally, MMP-3 (matrix metalloproteinase-3) and TIMP-1 (matrix metalloproteinase inhibitor-1) of synovium and cartilage tissue were measured by immunohistochemistry method. Results Compared to model group, continuous femoral nerve block could significantly relieve pain in KOA rabbits (P〈0.05 or P〈0.01). In experimental groups, synovial inflammation and cartilage damage were remarkably reduced; the expression levels of TNF-α, IL-1 and IL-6 were also evidently decreased (P〈0.05 or P〈0.01), which C, D and E group were more obvious (P〈0.01). In addition to, MMP-3 and TIMP-1 of synovium and cartilage tissue were significantly reduced in experimental groups (P〈0.05 or P〈0.01) , improving the imbalance MMP-3/TIMP-1. Conclusion Continuous nerve block can ease the pain response of KOA rabbits, and improve synovial cartilage damage. The mechanisms may be relative to reduce synovial fluid TNF-α, IL-1 and IL-6, as well as to improve the abnormal expression of MMP-3 and TIMP-1 of synovium and cartilage.
