MACC1和hTERT蛋白在胆管癌中的表达及其意义

摘要 目的探讨结肠癌转移相关基因1(MACC1)和人端粒酶逆转录酶(h TERT)蛋白在胆管癌中的表达及其意义。方法收集60例胆管癌、30例胆道良性病变患者术前血清和术后病理组织标本及30例体检健康者血清标本,分别采用免疫组化染色技术和ELISA法检测组织及血清中MACC1和h TERT蛋白的表达水平,并结合临床病理资料进行统计分析。结果 MACC1和h TERT蛋白在胆管癌患者组织和血清中的表达显著高于胆道良性病变组(P<0.01)。在胆管癌中,临床分期晚、伴有淋巴转移的癌组织中MACC1、h TERT蛋白表达较高(P<0.05)。胆管癌中MACC1蛋白与h TERT蛋白表达的一致性分析均具有良好的相关性(r=0.699,P<0.01)。血清中MACC1和h TERT单独和联合检测的ROC曲线下面积(AUCROC)分别为0.896、0.909、0.916,两者联合检测的敏感性最高(91.7%),MACC1单独检测的特异性最高(90.0%)。结论 MACC1和h TERT可以较好地鉴别胆道恶性肿瘤与良性病变。 Objective To investigate the expression and clinical significance of metastasis-associated in colon cancer-1 ( MACC1 ) and human telomerase reverse transcriptase (hTERT) in cholangiocarcinoma. Methods Preoperative serum and postoperative pathologic tissue samples from 60 cholangiocarcinoma patients and 30 patients with benign biliary duct lesions, and 30 serum samples from healthy controls were collected. The levels of MACC1 and hTERT in these tissues and serum samples were detected by immunohistochemistry and ELISA, respectively. Then, the associations of MACC1 and hTERT levels with clinical pathological data were analyzed. Results The expression levels of MACC1 and hTERT in pathological tissue and serum of cholangiocarcinoma patients were significantly higher than that in the patients with benign biliary duct lesions ( P 〈 0.01 ). Among the cholangiocarcinoma patients, the expression levels of MACC1 and hTERT in cancer tissues of the patients with later clinical stage and lymphatic metastasis were significantly higher than that without (P 〈 0.05 ), and the expression level of MACC1 was positively correlated to hTERT (r = 0. 699, P 〈 0.01 ). The areas under the receiver operating curve ( AUC^ROC ) of MACC1, hTERT and combination of MACC1 and hTERT for the diagnosis of cholangiocarci- noma were 0. 896, 0.909 and 0. 916, respectively. The highest sensitivity and specificity were found in the combination of MACC1 and hTERT (91.7%) and MACC1 alone (90.0%) , respectively. Conclusion The expression levels of MACC1 and hTERT may com- mendably distinguish malignant and benign biliary tract diseases.
