摘要 目的:通过病例-对照研究分析川南地区正常人群与糖尿病前期人群IL-1β基因rsl6944位点基因型及等位基因频率分布,探讨IL-1β基因多态性与糖尿病前期的相关性。方法:采用PCR—RFLP结合DNA测序的方法检测IL-1β基因rsl6944位点多态性,糖尿病前期376例,对照组377例,分析两组间基因型及等位基因频率的分布和临床生化指标与基因型之间的相关性。结果:IL-1β基因rsl6944位点CC、CT和1Tr基因型在糖尿病前期中的频率分别为30.9%、47.9%、21.3%,对照组为21.2%、53.8%、24.9%,两组比较差异有统计学意义(x2=9.119,P=0.010),等位基因C、T频率在糖尿病前期和对照组分别为54.8%、45.2%和48.1%、51.9%。以CT+TT基因型为对照,CC基因型发生糖尿病前期的风险增高(OR=1.656,95%CI:1.191~2.304,P=0.003)。葡萄糖耐量减退组与对照组相比差异具有统计学意义(X2=9.666,P=0.008),以CT+TT基因型为对照,CC基因型发生葡萄糖耐量减退的风险增高(OR=1.753,95%CI:1.227~2.505,P=0.002),而空腹血糖受损及混合组差异未见统计学意义。结论:IL-1β基因rs16944位点多态性与糖尿病前期发生具有相关性.CC基因型可能增加糖尿病前期的发病风险。 Objective To analyze interleukin 18 gene rs16944 genotype frequency and allele frequency in normal and pre-diabetic of southern Sichuan population and to study the relationship between interleukin-1β gene rs16944 polymorphism and pre-diabetic. Methods Interleukin-1β gene rs16944 polymorphism including 376 cases with pre-diabetic and 377 control subjects were identified by PCR-RFLP and DNA sequencing. The distribution of genotype and allele were analyzed to identify its correlation with clinical data and genotypes. Results Frequency of CC, CT and TF genotype in interleukin -1β gene rs16944 of pre-diabetic are 30.9%, 47.9% and 21.3% respectively and 21.2%, 53.8% and 24.9% respectively in control group (X2 = 9.119, P =0.010). Allelic gene C and TF frequency were 54.8% and 45.2% in pre-diabetic group, and 48.1% and 51.9% in control group. Compared with CT+TT genotype, CC genotype showed an increased risk to develop pre- diabetic, with OR = 1.656(95%CI: 1.191 - 2.304, P = 0.003). The difference of genotype in control group and impaired Glucose Tolerance group was statistically significant (X2 = 9.666, P=0.008). Compared with CT+ TF genotype, CC genotype showed an increased risk to develop impaired Glucose Tolerance, with OR = 1.753 (95%CI: 1.227 - 2.505, P = 0.002). But no significant differences were seen regarding impaired fasting glucose and mixed group. Conclusion There is correlation between intedeukin 18 gene rs16944 polymorphism and pre- diabetic. CC genotype could be a risk factor of pre-diabetic.
