急慢性踝关节韧带损伤的磁共振影像特点分析 被引量:11

摘要 目的探讨急、慢性踝关节韧带损伤的磁共振(MRI)影像特点。方法回顾性分析2014年9月至2016年2月于我院就诊并接受MRI检查的84例急、慢性踝关节韧带损伤患者临床资料,记录其受损踝关节韧带的MRI影像特点。结果 84例受试者中损伤1条韧带者68例,同时损伤2条韧带者14例,同时损伤3条韧带者2例;共检出102条韧带存在不同程度的损伤,其中内侧副韧带损伤51条(胫舟韧带损伤3条,胫距前韧带损伤46条,胫跟韧带损伤2条),外侧副韧带损伤48条(距腓前韧带损伤29条,距腓后韧带损伤5条,跟腓韧带损伤14条),胫腓韧带联合损伤3条(下胫腓前韧带损伤1条,下胫腓后韧带损伤2条)。急性损伤患者MRI图像多表现为韧带部分撕裂或完全断裂,边界欠清,T2WI序列图像及PD序列图像均可于病灶处见不均匀增高信号,病灶周围有高信号积液影或软组织水肿征;慢性损伤患者MRI图像则多表现为韧带不规则增粗或变细,信号不均匀。结论急、慢性踝关节韧带损伤的MRI影像各有其特点,临床可将MRI检查作为一种切实可行的影像检查手段用于诊疗工作中,为治疗方案的拟定提供依据。 Objective To investigate the magnetic resonance imaging (MRI) characteristics of acute and chronic injury of ankle ligament. Methods The clinical data of 84 patients with acute and chronic injury of ankle ligament, who were treated in our hospital between September 2014 and February 2016, were retrospectively ana- lyzed. The MRI characteristics of the injured ankle ligament were recorded. Results Among 84 cases, there were 68 cases with 1 injured ligament, 14 cases with 2 injured ligaments at the same time and 2 cases with 3 injured lig- aments at the same time; 102 ligaments were detected in different degrees of injury, including 51 injured medial collateral ligaments (3 injured tibionavicular ligaments, 46 injured anterior tibial ligaments, 2 injured tibiocalcaneal ligaments), 48 injured lateral collateral ligaments (29 injured anterior talofibular ligaments, 5 injured posterior peroneal ligaments, 14 injured caleaneofibular ligaments) and 3 injured tibial and fibular ligament (1 injured lower anterior tibial and fibular ligament, 2 injured lower tibial and fibular ligaments). MRI images of patients with a- cute injury mainly showed partial or total tear or rupture of ligament, unclear boundaries. T2WI images and PD images showed inhomogeneous increased signal in lesions, with high-signal effusion shadow or soft tissue edema sign; MRI images of patients with chronic injury mainly showed irregular thickening or thinning of the ligament, and the signal was inhomogeneous. Conclusion MRI imaging findings of acute and chronic injury of ankle liga- ment have their own characteristics, which can be used as a practical imaging examination in the diagnosis and treatment, providing basis for the development of treatment plan.
