摘要
目的 观察急性重症心肌炎大鼠心脏粘膜紧密连接蛋白(ZO-1)、闭锁蛋白(occludin)及白细胞介素(IL)-18的表达变化,探讨急性重症心肌炎时心脏屏障功能损伤的机制.方法 选择SD大鼠60只,随机分为三组,分别为对照组、假手术组、心肌炎组,每组各20只.心肌炎组制作急性重症心肌炎模型,假手术组大鼠麻醉后手术进腹,仅翻动心脏组织.对照组不进行腹部手术.术后24h处死各组大鼠,取心脏组织.光镜下观察心脏组织病理学改变,免疫组化检测各组心脏粘膜ZO-1、Occludin、IL-18蛋白的表达,实时荧光定量PCR检测各组心脏粘膜ZO-1、Occludin、IL-18 mRNA的表达.结果 光镜下可见心肌炎组心脏黏膜绒毛上皮坏死脱落,血管充血扩张,大量中性粒细胞浸润.对照组、假手术组、心肌炎组ZO-1阳性率分别为85%、70%、15%,心肌炎组ZO-1阳性率低于对照组及假手术组(P<0.05).对照组、假手术组、心肌炎组Occludin阳性率分别为75%、65%、20%,心肌炎组Occludin阳性率低于对照组及假手术组(P<0.05).对照组、假手术组、心肌炎组IL-18阳性率分别为15%、15%、80%,心肌炎组IL-18阳性率高于对照组及假手术组(P<0.05).心肌炎组ZO-1、Occludin mRNA表达水平均低于对照组及假手术组(P<0.05);,IL-18高于对照组及假手术组(P<0.05).结论 急性重症心肌炎时,紧密连接相关蛋白Z0-1、Occludin表达降低,IL-18表达增加,可能共同参与了急性心肌炎心脏粘膜屏障功能损伤的病理过程.
Objective To investigate the expression of tight junction protein zonula occludens (ZO-1),occludin and interleukin (IL-18) and the underlying mechanisms of barrier dysfunction in acute severe myocarditis.Methods 60 SD mice were randomly divided into three groups:control group,sham operation group and myocarditis group.Each group has 20 mice.The model of acute severe myocarditis was developed by myocarditis group.Sham operation groups were only treated with stirring of heart after anesthesia.Control group did not receive operation.Mice were killed and heart tissues were extracted in each group 24h after the operation.Pathological changes of heart tissues were detected under light microscope.Immunohistochemistry was used to detect the expression of ZO-1,occluding and IL-18.Real time polymerase chain reaction (RT-PCR) was used to detect the mRNA expression of ZO-1,occludin and IL-18.Results There were lots of necrosis in heart tissues with inflammatory cells infiltration.The positive rates of ZO-1,occludin and IL-18 were 85%,70% and 15%,75%,65 % and 20%,15 %,65% and 80% in control group,sham operation group and myocarditis group,respectively (P all 〈0.05).The rate of ZO-1 and occludin in myocarditis group was lower than control group and sham operation group,while the rate of IL-18 in myocarditis group was higher than control group and sham operation group.The mRNA expression of ZO-1 and occludin in myocarditis group were lower than control group and sham operation group (P 〈 0.05).IL-18 showed higher level as compared with control group and sham operation group (F =17.48,P 〈 0.05).Conclusion The expression of ZO-1 and occludin and were decreased in acute severe myocarditis,while IL-18 showed increased.The results revealed that ZO-1,occluding and IL-18 might involve in the pathogenesis of barrier function injury in acute severe myocarditis.
