摘要
目的:探讨孕期营养干预结合分娩期体位干预对剖宫产率的影响。方法:选择2015年7月~2016年7月在本院健卡的孕妇390例作为观察组,孕期给予营养干预和随访,产时选择自由体位分娩;另选同期在本院分娩、孕期未经过营养干预的孕妇390例作为对照组,在知情同意的情况下选择传统体位分娩。比较两组巨大儿、母体合并症及剖宫产率发生的情况。结果:母体合并症发生率观察组(产前10.7%、产后3.1%)低于对照组(产前20.7%、产后5.1%);胎儿合并症观察组(巨大儿6.4%、FGR3.3%)低于对照组(巨大儿12.8%、FGR 7.7%);剖宫产率观察组(24.1%)低于对照组(53.1%),差异均有统计学意义(P〈0.05)。结论:妊娠期个体化营养干预联合分娩期自由体位分娩可改善母婴结局,降低剖宫产率。
Objective: To explore the influence of gestational nutritional guiding and pregnant women adopting free position during delivery on rate of cesarean delivery. Method: From July 2015 to July 2016, 390 pregnant women had received nutritional guiding, and adopted free position during delivery were in the observation group. Another 390 pregnant women who hadn't received above intervention were in the control group. Rate of macrosomia, maternal complica- tions, and cesarean section were compared between the two groups. Result: In observation group, rate of maternal complication in observation group (pregnancy period was 10.7 % and postpartum was 3.1%) was significant lower than that in control group (pregnancy period was 20.7 % and postpartum was 5.1%). Rate of abnormal fetal weight in ob- servation group (macrosomia was 6.4% and FGR was 3.3%) was significant lower than that in control group (macro- somia was 12.8% and FGR was 7.7 %). Rate of cesarean delivery in observation group (24.1%) was significant lower than that in control group (53.1% ) (P〈0.05). Conclusion : Gestational nutritional guiding and pregnant women adop- ting free position during delivery can improve maternal and child outcomes, and reduce the rate of cesarean delivery.
