摘要
目的 比较CT增强检查中造影剂经单筒(碘海醇)和双筒(碘海醇+0.9%盐水)两种高压注射器方式注射时,碘造影剂对肌酐值及造影剂肾病发生率的影响,明确两种注射方式的优势。方法 根据纳入与排除标准选取2016年6-7月在我院行增强CT检查的患者116例,随机分为A组(单筒注射)57例、B组(双筒注射)59例,记录两组患者的基本特征及造影剂肾病(CIN)发生率及增强CT前24 h及增强CT后1 d、3 d、7 d的肌酐值并比较。结果 A、B两组患者基本资料比较差异无统计学意义(P〉0.05);A组CT增强后1 d、3 d、7 d的血肌酐值与造影前比较无明显差异(P〉0.05);B组CT增强后1 d、3 d、7 d的血肌酐值与造影前比较差异无明显差异(P〉0.05);A、B两组CT增强后各时间段血肌酐值比较差异无统计学意义(P〉0.05);A、B两组CIN发生率比较差异无统计学意义(P〉0.05)。结论 CT增强中碘造影剂经单筒或双筒注射时对肾功能影响均较小,考虑到单筒注射时伪影形成,对部分部位的图像质量略有影响,所以注射方式的选择应结合具体检查部位而决定。
Objective To compare the effect of iodine contrast agent on creatinine value and contrast-induced nephropathy via the injection of contrast agent by two high-pressure syringes of monocular(iohexol) and binocular (iohexol+ 0.9% saline) in CT enhanced examination, so as to understand the advantages of the two injection methods. Methods According to the criteria of inclusion and exclusion, 116 patients who underwent enhanced CT examination in our hos- pital from June to July 2016 were selected and randomly divided into group A (monocular injection method) of 57 cases, and group B(binocular injection method) of 59 cases. The basic characteristics and the incidence of contrast-induced nephropathy(CIN) were recorded in the two groups of patients. The values of creatinine in 24 h before enhancement, and 1 d, 3 d, 7 d after enhancement were compared within group A and group B, and values of creatinine were com- pared between groups corresponding to the number of days. Results There was no significant difference between the two groups in the basic characteristic data (P〉0.05); there was no significant difference in the serum creatinine value in 1 day, 3 days and 7 days after CT enhancement in group A(P〉0.05); there was no significant difference in the serum creatinine value in 1 day, 3 days and 7 days after CT enhancement in group B (P〉0.05); there was no significant difference of serum creatinine value in each time point after CT enhancement between the two groups(P〉0.05); the incidence of CIN in group A and B was not statistically significant(P〉0.05). Conclusion The effect of iodine contrast agent on renal func- tion is smaller in monocular and binocular syringes in CT enhancement. The formation of artifacts is considered in monocular injection, and part of the image quality is slightly affected, so the choice of injection methods should be combined with the specific parts of examination for final decision.
