摘要
目的分析2型糖尿病(type 2 diabetes mellitus,T2DM)患者胰岛素抵抗和胰岛B细胞功能的影响因素。方法统计T2DM患者行口服葡萄糖耐量试验(oral glucose tolerance test,OGTT)和胰岛素释放试验的结果,按照患者的年龄、病程、体重指数(body mass index,BMI)、糖化血红蛋白(glycosylated hemoglobin,HbA1c)水平、民族等因素分组,分析HbA1c、BMI、胰岛素释放量、胰岛B细胞功能、稳态模型评估的胰岛素抵抗指数(homeostasis model assessment of insulin resistance,HOMA-IR)在各组间的差异。结果 HOMA-IR、胰岛素释放量、胰岛B细胞功能随着病程进展呈逐渐下降趋势;胰岛素释放量、HOMA-IR、胰岛B细胞功能随着BMI的增加而逐渐升高;胰岛素释放量和胰岛B细胞功能随着HbA1c水平的升高而逐渐降低;维吾尔族患者胰岛素释放量和HOMA-IR偏高。HOMA-IR与T2DM患者的年龄、病程、HbA1c水平无关(P>0.05),与BMI相关(P<0.01)。胰岛B细胞功能与年龄、病程无关(P>0.05),与HbA1c水平和BMI相关(P<0.01)。结论 T2DM患者胰岛素抵抗与BMI相关,胰岛B细胞功能可能与BMI、HbA1c水平及民族有关。
Objective To analyze the influencing factor of insulin resistance and B-cell function in patients with type 2 diabetes mellitus (T2DM). Method The results of oral glucose tolerance test (OGTT) and insulin releasing test were statistically analyzed. The results were divide into groups by age, course, body mass index (BMI), glycosylated hemoglobin (HbAlc), nation and other factors, analyzed its relationship with BMI, HbAlc, insulin secretory volume, B-cell function, homeostasis model assessment of insulin resistance (HOMA-IR). Result HOMA-IR, insulin secretory volume and B-cells functiont gradually decreased with the progress of disease; insulin secretory volume, HOMA-IR, B-cells function increased gradually with the increase of BMI; insulin secretory volume and B-cells function with the increase of HbAlc gradually decreased; Uygur secretory volume and HOMA-IR was much higher; HOMA-IR was not related to age, course of disease, HhAlc (P 〉 0.05), related to BMI (P 〈: 0.01). B-cell functions was not related to age, course of disease (/9 〉 0.05), related to HbAlc and BMI (P 〈 0.01). Conclusion Insulin resistance in T2DM is associated with BMI, B-cell function may be related to BMI, HbAlc and nation.
