摘要
目的:分析火疗联合驱寒化瘀止痛膏治疗风湿寒性关节痛的临床疗效,从中医证候积分、血液流变学及生活质量评分三方面评价其疗效并探讨其影响机制。方法:将88例辨证为肝肾不足、寒湿瘀阻证的风湿寒性关节痛患者随机分为治疗组和对照组,对照组采用驱寒化瘀止痛膏热敷治疗,治疗组采用火疗联合驱寒化瘀止痛膏热敷,记录两组患者治疗前后在中医证候积分、血液流变学及生活质量评分的变化,观察临床疗效和不良反应。结果:对照组的总有效率为77.3%,治疗组为95.2%,治疗组总有效率高于对照组,差异有统计学意义(P<0.01);治疗后两组患者中医证候评分、生活质量评分均有所改善,差异有统计学意义(P<0.01),且治疗组明显优于对照组(P<0.01)。治疗过程中患者未发生不良反应。结论:火疗联合驱寒化瘀止痛膏可明显改善虚寒性关节痛患者的中医证候评分和血液流变学指标,提高生活质量,疗效明显优于单独使用驱寒化瘀止痛膏热敷。
Objective： To analyze the clinical curative effect of fire therapy combined with Quhan Huayu Zhitong paste in treating rheumatic cold joint pain, to evaluate the curative effect from the perspective of grade of TCM symptom, hemorheology and quality of life, and to explore the mechanism. Methods： 88 cases of patients with rheumatic cold joint pain of liver and kidney deficiency, dampness and blood stasis syndrome who were consistent with inclusion criteria, were randomly divided into the treatment group and the control group. Patients in the treatment group were treated with fire therapy combined with Quhan Huayu Zhitong paste and the patients in the control group were treated with Quhan Huayu Zhitong paste. The changes of grades of TCM syndrome, hemorheology and quality of life score were recorded before and after the treatment. The clinical efficacy and adverse drug reactions （ ADRs ） were observed. Results： The total effective rate of the treatment group and the control group was 95.2% and 77.3% respectively. The total effective rate of the treatment group was significantly higher than that of the control group, the difference between two groups was of statistical significance （P 〈 0.01）. The improvement of TCM symptom and quality of life of both groups were significant after treatment, the difference was of statistical significance （P〈 0.01）, the improvement of the treatment groupwas significantly higher than that of the control group. There was no adverse drug reactions （ ADRs ） in the treatment. Conclusion： Fire therapy combined with Quhan Huayu Zhitong paste could significantly improve the TCM symptom and hemorheology indexes in treating rheumatic cold joint pain, it could promote quality of life, its effect was better than that of the sole use of Quhan Huayu Zhitong paste.
