摘要
目的:建立可靠、稳定的手针针刺镇痛效应平台。方法:选择佐剂性关节炎疼痛模型为研究平台,先后以2种不同浓度的造模药物以及3种手针针刺刺激参数开展了研究。结果:采用低浓度佐剂(含结核杆菌浓度为1 mg/m L)造模后24 h大鼠致炎侧足底痛阈明显下降,并且可持续到观察周期结束(造模后21 d);高浓度佐剂(10 mg/m L)造模后24 h大鼠致炎侧足底痛阈亦明显下降,但在造模后容易出现足底溃疡等不良反应,影响指标的监测。针刺单侧足三里穴,未见明显的镇痛效应;针刺双侧足三里穴,留针30 min(其间每隔5 min行捻转手法,平补平泻,180次/min,持续2min)可有效抑制佐剂性关节炎大鼠的疼痛症状。结论:采用低浓度佐剂造模,手针针刺双侧足三里穴可有效建立针刺治疗佐剂性关节炎疼痛模型效应平台。在此效应平台的建立过程中,存在多种影响因素,主要包括实验动物、造模药物浓度、针刺干预参数、效应指标等因素,在具体研究过程中,应当综合考虑这些因素。
Objective : To establish the stable platform for studying the analgesic effect of manual acupuncture. Methods : We carried out this study with two different concentrations of adjuvant and three kinds of manual acupuncture stimulation parameters in adjuvant arthritis rats. Results : We found that thermal hyperalgesia was observed at 24 h after injecting low concentration ( 1 mg/mL) or high concentration ( 10 mg/mL) of adjuvant into the right hand paw, sustainable to the end of the observation period, but the high concentration of adjuvant could result in necrosis in the injection site after injection. Manual acupuncture at single Zusanli (ST36) had no analgesic effect, but manual acupuncture at bilateral ST36 (the needle was rotated at a rate of 3 spins per second for 120 s at intervals of 5 min, in a total of 30 min) showed better analgesic effect. Conclusion:Manual acupuncture at hilateral ST36 to treat inflammatory pain induced by low concentration of adjuvant could build the stable platform for studying the acupuncture effect on inflammatory pain model. There are many factors that influence the establishment of this platform, mainly including experimental animals, concentration of adjuvant, acupuncture stimulation parameters and outcomes. We should consider these factors during the study.
