摘要
目的比较结核性脑膜炎、细菌性脑膜炎、病毒性脑膜炎患者脑脊液中腺苷脱氨酶的含量和临床意义。方法采用回顾性分析把结核性脑膜炎患者62例、细菌性脑膜炎患者57例、病毒性脑膜炎患者55例分三组,运用酶显色法检测腺苷脱氨酶水平。结果结核性脑膜炎组、细菌性脑膜炎组、病毒性脑膜炎组腺苷脱氨酶水平分别是(15.73±5.64U/L)、(6.12±2.87U/L)、(3.24±1.44U/L);核性脑膜炎患者腺苷脱氨酶水平明显高于细菌性脑膜炎和病毒性脑膜炎患者(p<0.05);疗效好转的结核性脑膜炎患者结核性脑膜炎水平明显低于患者入院初期的水平,差异有统计学意义(P<0.05));疗效越好,结核性脑膜炎腺苷脱氨酶水平随之不断下降,细菌性脑膜炎、病毒性脑膜炎腺苷脱氨酶水平接近正常值,结核性脑膜炎与两者比较有统计学差异([微软用户3]P<0.05)。结论检测脑脊液中腺苷脱氨酶水平有利于结核性脑膜炎的鉴别诊断和提前发现,进行动态监测可以对治疗效果正确评估,为临床治疗提供指导意见。
Objective To compare the patients with tuberculous meningitis and bacterial meningitis, viral meningitis in the cerebrospinal fluid adenosine deaminase levels and clinical significance. Methods 62 patients with tuberculous meningitis, 57 patients with bacterial meningitis, 55 patients with viral meningitis in three groups were retrospective analyzed, using the method of enzyme color detection of adenosine deaminase levels. Results Tuberculous meningitis group, bacterial meningitis, viral meningitis group adenosine deaminase levels were(15.73 + 5.64), (6.12 + 2.87), (3.24 + 1.44) U/L; Patients with tuberculous meningitis adenosine deaminase levels significantly were higher than that of bacterial meningitis and viral meningitis patients (p 〈0.05); the better curative effect of tuberculous meningitis patients adenosine deaminase levels significantly were lower than that of the level of the early patients admitted to hospital, the difference was statistically significant (p 〈0.05); The better curative effect, adenosine deaminase levels lower (p 〈0.05); The level of Tuberculous meningitis group, bacterial meningitis were closed to normal value, there were statistically significant (1)〈0.05). Conclusion The detection of cerebrospinal fluid adenosine deaminase levels is helpful for the differential diagnosis of tuberculous meningitis and early found that dynamic monitoring treatment effects can be properly evaluated, and provide guidance for clinical treatment.
