增加翻转角提高钆塞酸二钠肝胆期图像对比度的研究 被引量:1

The Study of Increasing Flip Angle to Improve the Image Contrast on Hepatobiliary Phase of Gd-EOB-DTPA

摘要 目的研究增加翻转角(FA)对提高钆塞酸二钠(Gd-EOB-DTPA)肝胆期图像对比度的价值及影响。方法对54例患者分别行FA 10°、20°及30°的Gd-EOB-DTPA肝胆期成像,对不同FA间的肝脏相对强化值(LRE)、肝-脾比值(LSR)、肝-门静脉比值(LPR)、肝实质-肝占位比值(LLR)、胆管-胰腺比值(BPR)、胆管-肝脏比值(BLR)、腹壁及腹腔脂肪抑制比(FSR)进行对比分析。结果 FA为30°时LSR、LPR、LLR、BPR、BLR值最高,但腹腔和腹壁的FSR最低,FA为20°时LRE值最高,各指标不同FA组间比较均有统计学差异(P<0.05)。组内两两比较示,FA 10°和FA 30°的LRE无统计学差异(P>0.05),其余各组组内比较均有统计学差异(P<0.05)。此外,FA越大,LSR、LPR、LLR、BPR、BLR值越高,而FSR越低。结论增加FA能增加Gd-EOB-DTPA肝胆期肝实质、肝内病灶及胆管的对比度,但同时会降低腹壁和腹腔脂肪抑制效果。 Objective To assess the value and influencing factors of high flip angle to increase the image contrast on hepatobiliary phase of Gd-EOB-DTPA. Methods Fifty-four patients underwent Gd-EOB-DTPA-Enhanced MRI with hepatobiliary phase at 10°,20°,30° flip angles. To statistically analyze the indices of Liver Relative Enhancement( LRE),Liver-to-Spleen Ratio( LSR),Liver-to-Portal vein Ratio( LPR),Liver-to-Lesion Ratio( LLR),Biliary duct-to-Pancreas Raito( BPR),Biliary duct-to-Liver Ratio( BLR),and Fat-Suppression Ratio( FSR) of abdominal cavity and wall between each different flip angle. Results The maximal value of LSR、LPR、LLR、BPR、BLR and minimum value of FSR of abdominal cavity and wall were measured in FA30°,and the maximal value of LRE was measured in FA20°. There were statistical differences in the indices between each different flip angles,P〈0. 05. According to the LSD of One-way ANOVA analysis,there were statistical differences between each of the sub-groups except LRE between FA10° and FA30°. In addition,the value of LSR、LPR、LLR、BPR、BLR increased and FSR decrease with the increasing the flip angle. Conclusion Increasing the flip angle can strengthen the contrast of liver parenchyma,lesion and biliaryduct; conversely,it weakened the fatsuppression effect of the abdominal wall and cavity.
