摘要
目的：分析桥本甲状腺炎（Hashimoto thyroiditis,HT）的二维及多普勒超声表现。方法：回顾性分析我院2014年5月-2015年8月收治的30例桥本甲状腺炎患者临床资料,依据病理检查结果分为弥漫肿大型18例、结节肿大型8例、局限型4例,并分析其二维及多普勒超声表现。结果：二维表现如下：（1）甲状腺体积表现：弥漫肿大型与结节肿大型均表现为双侧甲状腺体积增大,前后径和峡部明显增厚;局限型甲状腺体积基本无肿大;（2）内部回声表现：弥漫肿大型内部回声降低,可见不规则网络样改变、结节肿大型内部回声不均且以低回声为主、局限型单侧或双侧甲状腺内存在1-2个不均匀低回声区。多普勒超声表现如下：弥漫肿大型血流信号显著增强,内径粗大,呈＂火海征＂、结节肿大型边缘可见环状或半环状血流信号,且信号丰富、局限型血流信号杂乱且明显增多。结论：分析桥本甲状腺炎的二维及多普勒超声表现可准确对其进行分型并为临床诊疗方案的制定与实施提供帮助。
Objective：To analyze Hashimoto thyroiditis（Hashimoto thyroiditis,HT）with two-dimensional and doppler ultrasonography.Methods：A retrospective analysis in May 2014 to August 2015 with 30cases of hashimoto thyroiditis were performed,including18 cases with diffuse large swollen,8cases withnodes swollen large,4cases withjust type,then the two-dimensional and doppler ultrasonography featureswere analyzed.Results：The two-dimensional performance was as follows：（1）The thyroid volume：diffuse large swollen and nodes swollen large were characterized by bilateral thyroid volume increases,anteroposterior diameter and isthmus of thyroid gland were significant thicken.But there was no swelling in limited type of thyroid volume;（2）Internal performance of the echo：internal echo was reduced and there were someirregular network changes in diffuse large swollen;uneven internal echo and give priority to with low echo were found in nodes swollen large;there were 1-2uneven keyed acoustic area in limited type unilateral or bilateral thyroid.Doppler sonographic findings are as follows：blood flow signal is significantly enhanced,inner diameter was bulky,as ＂fire＂ in diffuse swell large,circular edge visible or half circular blood flow signals were found in nodes swollen large,and signal type rich,but in limited type blood flow signal mixed and disorderly and increased obviously.Conclusion：Analysis of hashimoto thyroiditis of two-dimensional and doppler ultrasound can accurately carries on the classification and provide help for clinical diagnosis and treatment plan formulation and implementation.
