摘要
目的研究并比较不同方法检测梅毒螺旋体抗体的效果。方法随机选取2014年12月至2016年2月于本院确诊的362例梅毒患者为研究对象,均行甲苯胺红不加热血清试验(toluidine red untreated serum test,TRUST)、梅毒螺旋体明胶凝集试验(treponema pallidum particle agglutination,TPPA)及酶联免疫吸附测定(enzyme-linked immunosorbent assay,ELISA),比较3种检测方法的特异性、灵敏度、阳性检测率及Kappa系数。结果 3种检测方法的特异度和灵敏度比较均无显著差异(P>0.05)。TPPA和ELISA的阳性检测率(77.07%,79.28%)均高于TRUST(44.75%)(P<0.05)。TPPA和ELISA的Kappa系数(0.97±0.15,0.83±0.12)亦高于TRUST(0.35±0.09)(P<0.05)。TPPA的阳性检测率和Kappa系数与ELISA比较无明显差异(P>0.05)。结论临床上采用TRUST、TPPA及ELISA检测梅毒螺旋体抗体均具有一定的积极作用,其中TPPA和ELISA的检测结果可靠性更高。
Objective To study and compare the effect of different methods for detection of treponema pallidum antibody. Method 362 patients with syphilis in our hospital from December 2014 to February 2016 were selected as objects of study and all patients were detected with toluidine red untreated serum test (TRUST), treponema pallidum particle agglutination (TPPA) and enzyme-linked immunosorbent assay (ELISA), The specificity, sensitivity, positive detection rate and Kappa coefficient of the three test methods were compared. Result The detection results showed no significant differences in the specificity and sensitivity among the three test method (P 〉 0.05). The positive detection rate of TPPA and ELISA were respectively 77.07% and 79.28%, both higher than that of TRUST, which was 44.75%. Kappa coefficient of TPPA and ELISA were (0.97±0.15) and (0.83±0.12), both higher than TRUST, which was (0.35±0.09) (P 〈0.05). However, the positive detection rate and Kappa coefficient between TPPA and ELISA showed no significant difference (P〉0.05). Conclusion TRUST, TPPA and ELISA play certain positive role in the detection oftreponema pallidum antibody, in which TPPA and ELISA are more reliable and have significant application value.
