摘要
目的 探讨单纯全身麻醉与全麻复合硬膜外麻醉在老年单肺通气中对血流动力学的影响。方法 选取2013年5月~2015年3月收治并行胸科手术的85例老年患者为研究对象,根据患者麻醉方式分为全麻组(34例)和全麻复合组(51例),比较两组患者手术前、术中30 min及术毕的收缩压、舒张压、心率变化及拔管时间和清醒时间。结果 术前两组患者收缩压、舒张压及心率水平比较差异无统计学意义(P〉0.05),术中30 min、术毕全麻复合组患者收缩压、舒张压及心率水平均低于全麻组,差异有统计学意义(P〈0.001)。全麻复合组拔管时间及清醒时间均短于全麻组,差异有统计学意义(P〈0.001)。结论 全麻复合硬膜外麻醉在老年单肺通气中应用较单纯全麻更能有效维持患者血流动力学稳定,缩短患者拔管时间和清醒时间。
Objective To investigate the effects of general anesthesia and combined general and epidural anesthesia on hemodynamics in elderly patients with one-lung ventilation.Methods 85 elderly patients admitted to hospital and underwent thoracic surgery from May,2013 to March, 2015 were selected as research objects.All patients were divided into general anesthesia group (34 cases) and combined general and epidural anesthesia (51 cases) according to anesthetic ways. Systolic blood pressure, diastolic blood pressure and change of heart rate before operation, 30 min during operation and after operation as well as extubation time and awake time were compared between the two groups.Results Difference of systolic blood pres- sure,diastolic blood pressure and heart rate levels of the two groups before operation was not statistically significant (P〉 0.05) .Systolic blood pressure,diastolic blood pressure and heart rate levels of the combined general and epidural anesthesia group 30 min during operation and after operation were all significantly lower than those of the general anesthesia group, difference was statistically significant(P〈0.001 ).In addition, both extubation time and awake time of the combined general and epidural anesthesia group were shorter than those of the general anesthesia group, difference was statistically Significant (P〈0.001) .Conclusion Compared with general anesthesia, combined general and epidural anesthesia can more effectively maintain hemodynamic stability in elderly patients with one-lung ventilation, shorten extubation time and awake time.
