摘要
目的探讨微信方式延续护理模式在哮喘患者居家康复中的应用。方法挑选该院281例出院的哮喘患者分为观察组141例和对照组140例,二组患者出院时均给予常规出院指导,对照组出院后不进行任何护理干预;观察组在常规健康指导的基础上于出院时至6个月期间,通过微信群与患者进行沟通和健康指导,利用患者复诊时进行问卷调查。比较二组患者6个月后健康知识掌握情况、生活质量得分、哮喘急性发作次数、再入院率和患者满意率。结果观察组患者健康知识掌握情况、生活质量及满意度均显著优于对照组,哮喘急性发作次数及再入院率观察组均明显低于对照组(P<0.05)。结论微信方式的延续护理高效、快捷,能够实时对居家哮喘患者进行有效的健康指导,丰富了患者的哮喘知识,提高其生活质量和对医院服务的满意度,有效降低了哮喘的急性发作次数和再入院率。
Objective To explore the application of transitional care model in patients with asthma via WeChat after discharge.Methods Patients with asthma receiving routine discharge instructions when discharged from author's hospital were divided into two groups,the treatment group(n=141)and the control group(n=140).Patients in the control group received no additional care while patients in the treatment group will be instructed via WeChat for 6 months after discharge.A questionnaire was conducted during follow-up.Knowledge of health,quality of life scores,occurrence of acute asthma,re-admission and patients' satisfaction were compared after 6 months.Results Knowledge of health,quality of life scores and patients' satisfaction in the treatment group were superior to that in the control group.And occurrence rate of acute asthma and re-admission in the treatment group were significantly lower than that in the control group.Conclusions Usage of transitional care in patients with asthma via WeChat possesses advantages such as providing effective,convenient and real time care for patients,imparting knowledge about asthma,increasing quality of life and satisfaction to service from hospital and lowering occurrence rate of acute asthma and re-admission.
