摘要
目的:自拟疏肝祛毒汤联合血液净化对重症肝炎患者肝功能及基础临床指标代谢的影响。方法:将2013年5月~2015年5月医院收治的重症肝炎患者68例采用随机数字表法分为对照组和观察组,每组各34例,对照组采用人工肝辅助装置血浆置换(PE)联合连续性静-静脉血液滤过(CVVH)治疗,观察组在对照组基础上给予自拟疏肝祛毒汤治疗,30日为1个疗程。治疗1个疗程后评估疗效,对主症状进行量化评分,测定血清白蛋白(ALB)、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆固醇(TCH)、总胆汁酸(TBA)、凝血酶原活动度(PTA)、白介素8(IL-8)、白介素10(IL-10)。结果:观察组治疗有效率为88.24%略高于对照组的82.35%,但两者无明显差异(P>0.05);观察组治疗后乏力、肋痛、腹胀评分明显低于对照组(P<0.05);观察组治疗后ALT、AST、TBA、IL-8低于对照组,TCH、PTA、IL-10高于对照组(P<0.05)。结论:自拟疏肝祛毒汤联合PE、CVVH治疗重症肝炎效果确切,减轻机体炎性反应,改善TCH、PTA水平,减轻肝功能损害。
Objective： To study the effects of Self-designed Shugan Qudu Decoction combined with blood purification on liver function and basic clinical index metabolism in patients with severe hepatitis. Methods： 68 patients with severe hepatitis treated in our hospital between May 2013 and May 2015 were randomized into controlled group and the observation group by the random number table method,34 cases in each group. The controlled group was treated with plasma exchange（ PE） with the Artificial Liver Support System combined with continuous veno-venous hemofiltration（ CVVH）. On the basis,the observation group was treated with Self-designed Shugan Qudu Decoction. 30 days was 1course of treatment. After a course of treatment,the curative effect was evaluated. The quantitatively scoring was performed for the main symptoms. The serum albumin（ ALB）,alanine aminotransferase（ ALT）,aspartate aminotransferase（ AST）,total cholesterol（ TCH）,total bile acid（ TBA）,prothrombin activity（ PTA）,interleukin 8（ IL- 8） and interleukin 10（ IL- 10） were determined. Results： The response rate of treatment in the observation group（ 88. 24%） was slightly higher than that in the controlled group（ 82. 38%）,but there was no significant difference between the two groups（ P〈0. 05）; after treatment,the scores of fatigue,rib pain and abdominal distension in the observation group were significantly lower than those in the controlled group（ P〉0. 05）; after treatment,ALT,AST,TBA and IL- 8in the observation group were lower than those in the controlled group,while TCH,PTA and IL- 10 were higher than those in the controlled group（ P〈0. 05）. Conclusion： The effect of Self-designed Shugan Qudu Decoction combined with PE and CVVH in the treatment of severe hepatitis is exact,which can reduce the inflammatory reactions,improve the TCH and PTA levels and reduce the damage to liver function.
