摘要
目的了解兰州地区感染性腹泻婴幼儿中A组轮状病毒(RV)腹泻流行情况。方法 2009年1月至2014年6月在兰州大学第一医院儿科收集腹泻患儿粪便标本1 245份,采用酶联免疫吸附试验(ELISA)检测A组RV抗原,阳性标本采用逆转录-聚合酶链式反应(RT-PCR)进行基因分型。结果 1 245份标本中共检测出A组RV抗原608份,阳性率为48.84%。A组RV发病高峰为每年10月至次年2月。对608份A组RV阳性标本进行G、P分型,G分型以G3型为主,占23.68%(144/608),其次为G9、G2、G1和G4型,分别占14.14%(86/608)、7.24%(44/608)、7.07%(43/608)和0.82%(5/608)。P分型以P[8]、P[9]和P[4]型常见,分别占34.52%(165/478)、19.25%(92/478)和14.23%(68/478)。最常见的G、P型别组合为G8P[9]、G9P[8]和G3P[8],分别占25.94%(90/347)、19.02%(66/347)和17.29%(60/347);其次是G2P[4]、G1P[8]、G8P[3]和G3P[4],分别占10.09%(35/347)、7.20%(25/347)、4.61%(16/347)和4.03%(14/347)。结论 A组RV是兰州地区婴幼儿腹泻的常见病原,流行毒株呈现型的多样性变化。目前上市的A组RV疫苗适合兰州地区患儿使用,将G9型引入现有疫苗株具有重要的现实意义。
Objective To study the molecular epidemiology of group A rotavirus(RV)diarrhea in infants and young children in Lanzhou of China. Methods Stool samples were collected from 1 245 infants and young children with acute diarrhea who were seen at the Pediatric Ward of the First Hospital of Lanzhou University from January 2009 to June 2014.Group A rotavirus were detected by ELISA,further strain characterization of rotavirus was carried out by RT-PCR. Results A total of 608 stool samples were detected for group A rotavirus positive(48.84%).The peak season was from October to February of the following year.Among the 608 positive strains,G3 was the most common G genotype(144/608,23.68%),followed by G9(14.14%),G2(7.24%),G1(7.07%)and G4(0.82%).P[8],P[9]and P[4]were the most common P genotypes that accounted for 34.52%(165/478),19.25%(92/478)and 14.23%(68/478),respectively.The most common G/P genotype combinations were G8P[9],G9P[8]and G3P[8]which accounted for 25.94%(90/347),19.02%(66/347)and 17.29%(60/347),respectively,followed by G2P[4](10.09%),G1P[8](7.20%),G8P[3](4.61%)and G3P[4](4.03%),respectively. Conclusions Group A rotavirus was a major pathogen of diarrhea in infants and young children in Lanzhou of China.The virus variation of the epidemical strains were multiplex.Currently licensed group A rotavirus vaccines are suitable for the Children in Lanzhou.The introduction of effective G9 vaccines is very important.
