重组人钠脲肽联合左西孟旦对急性失代偿心力衰竭患者LVEF和BNP水平的影响 被引量:8

Effects of recombinant human brain natriuretic peptide combined with levosimendan on LVEF and BNP levels in patients with acute decompensated heart failure

摘要 目的:探讨重组人钠脲肽联合左西孟旦对急性失代偿心力衰竭患者左室射血分数(LVEF)及脑尿钠肽(BNP)水平的影响。方法:将68例急性失代偿心力衰竭患者分为两组,每组34例。对照组采用左西孟旦治疗,研究组在对照组基础上给予重组人钠脲肽治疗;对比两组治疗后心功能改善情况。结果:两组患者治疗后BNP、醛固酮、肾上腺素、血管紧张素Ⅱ水平显著降低(P<0.05),LVEF、每搏输出量(SV)水平显著提高(P<0.05);研究组治疗后BNP、醛固酮、肾上腺素、血管紧张素Ⅱ水平显著优于对照组,差异有统计学意义(t=2.133、6.212、4.055、2.969、2.284、5.999,P<0.05);研究组治疗后LVEF、SV水平显著高于对照组,差异有统计学意义(t=3.705、3.217,P<0.05);两组患者在治疗过程中均无明显不良反应发生,肝肾功能指标均未出现明显异常,无死亡病例出现。结论:重组人钠脲肽联合左西孟旦治疗急性失代偿心力衰竭,能显著提高患者的LVEF,降低BNP水平,改善心功能。 Objective: To investigate the effects of recombinant human brain natriuretic peptide combined with levosimendan on left ventricular ejection fraction(LVEF) and brain natriuretic peptide(BNP) level in patients with acute decompensated heart failure. Methods: 68 patients with acute heart failure were divided into two groups with 34 cases in each group. The control group was treated with levosimendan. The treatment group was given treatment with recombinant human brain natriuretic peptide. The improvement of cardiac function after treatment in the two groups was compared. Results: After treatment, the levels of BNP, aldosterone, epinephrine and angiotensinⅡ were significantly decreased in both groups(P 0.05). After treatment, the levels of BNP, aldosterone, epinephrine and angiotensi n Ⅱ in the treatment group were significantly higher than those in the control group. Statistical analysis showed significant difference(t=2.133, 6.212, 4.055, 2.969, 2.284, 5.999, P0.05). LVEF and stroke volume(SV) levels were significantly increased in the two groups after treatment(P0.05). After treatment, the levels of LVEF and SV in the study group were significantly higher than those in the control group. The difference is statistically significant(t=3.705 and 3.217, P 0.05); no obvious adverse reaction was observed in both groups during the course of treatment and hepatic and renal function were not obviously abnormal. No deaths occurred. Conclusion: Recombinant human brain natriuretic peptide and levosimendan treatment of acute decompensated heart failure can significantly improve the patient's left ventricular ejection fraction(LVEF), reduce BNP level, and improve heart function.
