摘要
目的探讨微量定点注射技术在改善面中部软组织松垂中的应用方法及治疗效果。方法设计面中部4点为注射点:颧弓中点、颧骨最高点、眶下孔外侧0.5 cm、笑肌中外1/3交点。注射过程中用肌电设备监测相关肌肉的变化。应用3D数字扫描技术记录治疗前后外观,分析相关数据,以评估面中部软组织的改变。结果本组共58例患者,术后随访6~12个月(平均8.5个月),有效随访42例;注射4点过程中相关肌肉均有电位改变;术后即刻及1~3个月随访时均发现鼻唇沟变浅、面中部软组织提升,面部呈现自然、和谐、美感的年轻化改善;3D扫描分析发现,自术后6个月起,随透明质酸吸收,提升效果逐渐出现减弱趋势。结论面中部4点的选择以肌肉附着点及走行为理论基础,使得注射提升精准、有效;加强型透明质酸为软组织提升提供了很好的支撑;该注射技术获得了很高的患者满意度,值得推广应用。
Objective To explore and evaluate the technology of midfacial fixed-point injection with microscale reinforced hyaluronic acid for improving soft tissue sagging. Methods Four points in the midface were selected for injection, including the midpoint of the zygomatic arch, the highest point of the zygomatic bone, 0.5 cm lateral to the infraorbital foramen, and 1/3 mediolateral intersection point of risible muscle. The electromyography was used for monitoring zygomalic major and minor muscle. 3D digital scanning was used to record the appearance pre- and post-injection. Results Fifty-eight female patients were admitted in this study. Forty-two cases were effectively followed-up for 6 to 12 months (average 8.5 months). Changes in target muscle potential were monitored by electromyography before and after 4-point injection. The nasolabial groove became shallow, the soft tissue of the midfaee uplifted immediately and 1 to 3months after injection. Since 6 months after operation, 3D scan showed that the lifting effect gradually weakened with the absorption of hyaluronic acid. Conclusions The choice of four points in the middle of the face is based on the muscle attachment point and walking behavior theory, which makes the injection enhancement accurate and effective. Enhanced hyaluronic acid provides a good support for soft tissue enhancement. The injection technique achieved natural, harmonious, aesthetic results and high patient satisfaction. It should be considered for popularization and application in clinic.
