摘要
目的:探讨多层面螺旋CT三维重建在诊断良性和恶性脊柱骨折中的价值。方法:回顾性分析2013年1月至2016年6月河北省唐山市玉田协和医院收治的120例脊柱骨折患者的临床资料,包括良性和恶性脊柱骨折患者各60例,收集多层面螺旋CT三维重建、核磁共振成像(MRI)和数字X线摄影技术(DR)影像学检查结果,并以病理结果作为金标准,评价3种影像学检查结果与病理检查结果的一致性,并计算灵敏度、特异度和Youden指数。结果:多层面螺旋CT三维重建的诊断正确率为85.83%,Kappa系数为0.717(P<0.001),高于MRI和DR的Kappa系数(0.533、0.067)。CT三维重建诊断脊柱骨折的灵敏度(83.33%)、特异度(88.33%)和Youden指数(71.67%)均高于MRI和DR。结论:多层面螺旋CT三维重建可对脊柱骨折进行"一站式检查",明确病变范围,鉴别脊柱骨折的良、恶性,值得临床推广应用。
Objective:To assess the value of application of multi-slice spiral CT (MSCT) with three dimensional (3D) reconstruction in the diagnosis of benign and malignant vertebral fractures. Methods: A retrospective analysis were conducted on 120 cases of spinal fractures (60 benign and 60 malignant) who were treated in our hospital from January 2013 to June 2016. The imaging data obtained from MSCT scanning with 3D reconstruction, magnetic resonance imaging (MRI) and digital X-ray radiography system (DR) were collected and compared with the pathology results. Results. The accuracy of MSCT inspection was higher than that of MRI and DR ( P〈0.05). In the diagnosis of benign spinal fractures, the sensitivity ranking was MSCT〉MRI〉DR, and the specificity ranking was MRI〉 MSCT〉DR. There were significant differences among MSCT, MRI and DR ( P %0. 05). In the diagnosis of malignant vertebral fracture, the sensitivity ranking was CT〉MRI〉DR, while the specificity ranking was MRI〉MSCT 〉DR. There were also significant differences among MSCT, MRI and DR (P 〈0.05). Conclusion. MSCT with 3D reconstruction could be a "one-stop inspection" for spinal fractures, and it is worthy of clinical application.
