摘要
目的完善已经使用了五年之久的某大学手术分级目录,规范手术分级管理,更好地适应手术科室的发展及对外交流。方法将某大学手术分级目录中23个专业4238个手术项目分别与原卫生部《手术分类目录(2011年版)(征求意见稿)》和《吉林省三级四级内镜诊疗技术目录》中的手术分级级别进行比较,并将其中266项手术级别高于国家级和省级目录的手术项目在临床科室进行调研。结果调研的全部手术项目的同意降级率为34.95%,比例比较低。调研的26个科室中,仅有6个科室的同意降级率为100%,有9个科室完全不同意降级,在这9个科室调研的手术项目占全部不同意降级手术项目的89.05%。结论手术分级目录应随医疗机构手术技术的不断提升,定期进行修订与完善,适时降低手术分级级别,既有利于规范手术分级管理,避免医师超权限手术;建议手术科室注重内涵质量和人才梯队建设,保障医疗质量和安全,同时也有利于培养青年医师,为青年医师和医院未来的发展奠定坚实的基础。
Objectives To standardize operation classification management,improve the use of five years of one university ＂Operation Classification Directory＂,make it adapt to the development of surgical departments and foreign exchange better.Methods To make a comparison on the operations levels among the ＂ Operation Classification Directory＂ 23 professional 4238 operation projects and the former Ministry of Health ＂Operation Classification Directory（2011 Edition）（Exposure Draft） ＂ and ＂Three and Four Level of Endoscopic Diagnosis and Treatment Technology Directory in Jilin Province＂,and conduct research on the 266 operation projects that higher than the national and provincial directory for research in clinical departments.Results The rate of the total surgical items agreed to downgrade was 34.95%.Research of the 26 departments,only 6 departments agreed to downgrade rate was 100%.There are 9 departments do not agree with the downgrade,and in the 9 departments of the surgical project accounted for 89.05%of the total did not agree with the downgrade surgery.Conclusions ＂Operation Classification Directory＂ should be revised and improved regularly and reduce grading timely with the continuous improvement of surgical techniques in medical institutions.It is not only conducive to the standardization of operation management,to avoid physicians over access surgery,and ensure the quality and safety of medical treatment.At the same time,it is also conducive to the training of young doctors,to lay a solid foundation for the future development of young physicians and hospitals.
