摘要
目的观察针药结合康复训练综合疗法治疗中风后肩手综合征的临床疗效。方法将90例中风后肩手综合征患者随机分为针药结合康复训练组和康复训练组,针药结合康复训练组45例采用电针、中药结合康复训练的综合方案治疗,对照组45例给予单纯康复训练治疗,观察2组治疗前及治疗4周、治疗8周、随访6个月时手肿胀程度、上肢疼痛VAS评分、简化FMA上肢运动功能评分、Barthel指数评分。结果 2组治疗4周、治疗8周、随访6个月时手肿胀程度、上肢疼痛VAS评分均明显降低(P均<0.05),简化FMA上肢运动功能评分、Barthel指数评分均明显升高(P均<0.05),且治疗组各指标改善情况均明显优于对照组(P均<0.05)。结论针药结合康复训练综合疗法治疗中风后肩手综合征疗效更好,可明显改善上肢运动功能,提高日常生活能力。
Objective It is to observe the clinical effect of acupuncture and medicine combined with rehabilitation therapy on shoulder-hand syndrome（ SHS） after stroke. Methods Ninety cases of patients with SHS after stroke were randomly divided into two group, 45 cases in the treatment group and 45 cases in the control group. The treatment group was treated with acupuncture and medicine combined with rehabilitation therapy, the control group was treated with rehabilitation therapy only. The main clinical effects indicators included hand swelling degree, limb pain Visual Analogue Scale （VAS） , simplified Fugl- Meyer assessment （sFMA） score, Barthel index （BI） score were observed before treatment, at 4 weeks, 8 weeks after treatment and at 6-month following-up in both groups. Results The hand swelling degree, VAS score, sFMA score and BI score of patients with SHS were improved significantly in both groups at 4 weeks, 8 weeks after treatment and at 6-month following-up （F 〈 0. 05 ） , and the improvements were more significant in the treatment group than that in the control group （P 〈0. 05）. Conclusion Acupuncture and medicine combined with rehabilitation therapy has preferable effects on SHS after stroke, which could obviously improve the patients^upper limb function and activity of daily living.
