摘要 [目的]明确植入式静脉输液港(VPA)与经外周穿刺置入中心静脉导管(PICC)对乳腺癌病人的应用效果。[方法]检索中国期刊全文数据库、中国科技期刊全文数据库、中国生物医学文献数据库、万方数据库、PubMed、Web of Science和Elsevier Science,收集2015年之前国内公开发表的关于植入式静脉输液港与PICC对乳腺癌病人应用效果比较的相关文献资料。并通过"滚雪球"方式,追查纳入文献的后附参考文献。应用RevMan5.3软件进行分析。[结果]共纳入16篇文献,Meta分析结果显示:采用VPA化疗的病人并发症明显少于PICC组;两种置管方式一次性置管成功率比较无统计学意义;静脉输液港的留置时间要明显比PICC长,而且采用静脉输液港化疗的乳腺癌病人的生活质量明显好于PICC病人。[结论]对于需长期化疗的乳腺病人,推荐使用VPA。护理人员要加强导管的护理,以减少病人并发症的发生,提高病人生活质量。 Objective:To identify the effects of venous port access(VPA) and PICC for patients with breast canc-er. Methods: The related document literature on the effects of VPA and PICC on breast cancer patients published before 2015 were searched and collected from databases including : China National Knowledge Infrastructure (CNKI),Wanfang Periodical Databases, Chinese biomedicine literature database ( SinoMed), Chinese Science Technology Periodical Databases (VIP) , PubMed, Web of science and Elsevier Science. RevMan 5. 3 software was used for analysis through the snowball approach,tracing into the literature attached to the references. Re-sults: A total of 16 articles were included. Meta - analysis showed that the complications of VPA chemotherapy were significantly less than those of PICC. There was no statistically significant difference in disposable catheter success rate between the two methods. Indwelling time of VPA was significantly longer than PICC, and the quality of life(QOL) of breast cancer patients treated with VPA was significantly better than that of patients treated with PICC. Conclusions: VPA is recommended for breast cancer patients who need long - term chemo-therapy. Nursing personnel staffs should strengthen the care of catheter to reduce the incidence of complica-tions, and improve QOL of patients.
